OBJECTIVETo reveal the the roles and clinical significance of Copper/Zinc Superoxide dismutase (Cu/Zn-SOD), Manganese Superoxide dismutase (Mn-SOD) activity and Malondialdehyde (MDA) concentration in adrenal grands and serum of patients with adrenal disease, we study the distribution of Cu/Zn-SOD and Mn-SOD in adrenal gland by immunohistochemistry and SOD activity and MDA concentration in serum by serology respectively to compare difference among adrenocortical adenoma, adrenocortical hyperplasia, pheochromocytoma patients and normal people, which may provide an new differential diagnosis for adrenal disease.METHODS1. The expression of Cu/Zn-SOD, Mn-SOD activity and MDA concentration in serum of patients with adrenocortical adenoma, adrenocortical hyperplasia, pheochromocytoma, adrenal medullary hyperplasia and normal people.The study population consisted of 33 patients with adrenal disease,including 12 cases of adrenocortical hyperplasia,10 patients with adrenocortical adenoma,11 cases of pheochromocytoma and 11 controls. We measured the concentration of MDA through the method of Thiobarbituric acid-reaction,the activity of SOD by the method of xanthine oxidase in the serum of all the participants before surgical treatment.RESULTSâ‘ SOD:The activity of Cu/Zn-SOD and Mn-SOD in study group were significantly lower than that of controls (P<0.05),Among the study group, the activity of Cu/Zn-SOD in patients with adrenocortical adenoma were the lowerst of all (P<0.05),whil the activity of Mn-SOD in patients with adrenocortical adenoma and pheochromocytoma were lower than that of adrenocortical hyperplasia (P<0.05).â‘¡MDA:The concentration of MDA in study group were significantly higher compared to that of controls (P<0.01),Among the study group, the concentration of MDA in patients with adrenocortical adenoma and pheochromocytoma were lower than that of adrenocortical hyperplasia (P<0.05)2. The expression of Cu/Zn-SOD, Mn-SOD activity in adrenal glands of patients with adrenocortical adenoma, adrenocortical hyperplasia, pheochromocytoma, adrenal medullary hyperplasia and normal people.To collect 44 conventional paraffin wax tissues of adrenocortical adenoma, adrenocortical hyperplasia, pheochromocytoma and normal adrenal from our hospital from January 2008 to March 2009 and normal adrenal corned from corpse. including 12 cases of adrenocortical hyperplasia,10 patients with adrenocortical adenoma,11 cases of pheochromocytoma and 11 contols. Immunohistochemical technique was applied to detect the expression of Cu/Zn-SOD and Mn-SOD in routinely processed tissue specimens from human adrenal neoplasm and hyperplasia. All tissues were fixed by formalin, its concentration is 10 percentage. Fixed time is between eighteen hours and twenty-four hours. Specimens were embedded by ozocerite and chip to Sum thickness. One specimen was HE stain, another was Cu/Zn-SOD and Mn-SOD immunoreactivity. Normal adrenal was positive contrast and PBS replaced first antibody was negative contrast. Positive matter was found in cytolymph and its colour was brown. Judgement standard is percentage of positive cell. The destination was to study the characteristic of Cu/Zn-SOD and Mn-SOD in adrenal by immunohistochemistry and statistics percentage of colouer positive cell to compare difference to adrenocortical adenoma,adrenocortical hyperplasia, pheochromocytoma and normal adrenal.RESULTSCu/Zn-SOD immunoreactivity was regularly seen in adrenocortical adenoma, adrenocorticohyperplasia and normal adrenal cortex, Cu/Zn-SOD expression was lower in adrenocortical adenoma tissue than that of adrenocorticohyperplasia and normal cortex (P<0.05).but neither in the pheochromocytoma, pheochromocytoma nor in the normal adrenal medulla. Mn-SOD positive cell were showed in adrenocortical adenoma, adrenocorticohyperplasia, normal adrenal cortex,and there was a significant difference among the three(P<0.05). Mn-SOD expression was lower in adrenocortical adenoma tissue than that of adrenocorticohyperplasia, normal adrenal cortex. Besides,Mn-SOD immunoreactivity was regularly seen in normal adrenal medulla and pheochromocytoma,and there was a significant difference between them(P<0.05).3.The correlation between SOD and MDA in adrenal tissue and serumBoth Cu/Zn-SOD and Mn-SOD activity in serum of patients with adrenocortical hyperplasia, adrenocortical adenoma and pheochromocytoma showed a significant negative with the concentration of MDA (Cu/Zn-SOD:r=-0.859,r=-0.831,r=-0.644, P<0.05; Mn-SOD:r=-0.817,r=-0.886,r=-0.714, P<0.05).The expression of Cu/Zn-SOD in tissu have a significant positive correlation with serological expression in study group (r=0.823,r=0.821, P<0.05),the same correlation was found in the expression of Mn-SOD as well (r=0.934,r=0.803,r=0.804, P<0.05)CONCLUSION1. There is obvious oxidative stress in patients with adrenal disease,reflects as the increased level of oxidizing metabolite (MDA)and the decreased activity of Cu/Zn-SOD and Mn-SOD as an antioxidants.This unbalance between the oxidants and antioxidants may as an important factor of adrenal disease.2. The differences in the expression of Cu/Zn-SOD and Mn-SOD activity among adrenocortical adenoma, adrenocortical hyperplasia, pheochromocytoma maybe correlation with the hormone release of adrenal glands, which provide an new differential diagnosis of adrenal cortical and hylic diseases.
