Obejective:Angiopathy is the main and chronic complications of diabetes, in which macrovascular disease is the important reasons for death of diabetes. To obeserve the change of serum ferritin among the subjects with normal control(NC), impaired glucose tolerance(IGT), type 2 diabetes without complications(DM-CAD) and type 2 diabetic macroangiopathy(DM+CAD). And meanwhile to determine the relationgships between the serum ferritin levels and factors including age,gender body mass index(BMI),blood pressure,fasting plasma glucose(FPG),fasting insulin(FIN),HbA1C,blood lipid. So as to further explain the role of iron load in the development of diabetes.MethodsMessuring glucose tolerance test snd general bochemical test, history taking and physical examinations. The serum ferritin was detected by ELISA in 22 heathy controls, (NC),23 of impaired glucose tolerance(IGT),24 of type 2 diabetes without complications(DM-CAD) and 22 of type 2 diabetic macroangipathy patients(DM+CAD).The levels of serum ferritin were detected by ELISA. The levels of blood glucose lipid,were detected by enzymology method. FINS were detected by radioimmunoassay(RIA). Blood sugar ferrohemoglobin(HbA1C) was measured by aminophenylborate gel affinity chromatography. All analysis were performed with SPSS 13.0 software package.Results:1,The serum ferritin was (82.93±25.22)ng/ml in NC subjects, (123.12±47.29) ng/ml in IGT subjects, (200.01±64.29)ng/ml in DM-CAD subjectes and (269.98±137.56)ng/ml in DM+CAD subjects. The serum ferritin levels of DM+CAD was higher than that in DM-CAD, NC and IGT. Its concentration is gradually increased and have statistical significance(P<0.05). Compared with NC and IGT, the serum ferritin levels was higher in DM-CAD subjects(both P<0.05). compared with NC, the serum ferritin level was higher in IGT. (P<0.05) 2,The total cholesterol levels were (4.01±0.60)mmol/L in NC subjects, (4.66±1.39)mmol/L in IGT subjects, (5.36±1.54)mmol/L in DM-CAD subjectes and (5.12±1.24)mmol/L in DM+CAD subjects. The total cholesterol level of NC was lower than that in IGT, DM-CAD, DM+CAD and have statistical significance(P<0.05). But among the three groups,there have no statistical significance(P>0.05)3,The fasting insulin levels were (8.78±3.01)mIU/L in NC subjects, (15.23±5.16)mIU/L in IGT subjects, (14.57±6.98)mIU/L in DM-CAD subjectes and (14.35±7.43)mIU/L in DM+CAD subjects. The fasting insulin level of NC was lower than that in IGT, DM-CAD, DM+CAD and have statistical significance(P<0.05). But among the three groups,there have no statistical significance(P>0.05)4,The fasting plasma glucose levels were (4.53±0.48)ng/ml in NC subjects,(5.23±0.52)ng/ml in IGT subjects, (8.43±2.70)ng/ml in DM-CAD subjectes and (8.66±2.50)ng/ml in DM+CAD subjects. The fasting plasma glucose level of NC was lower than that in IGT, DM-CAD, DM+CAD and have statistical significance(P<0.05). Compared with DM-CAD and DM+CAD groups, the fasting plasma glucose level was lower in IGT group with statistical significance(P<0.05), while there have no statistical significance between DM-CAD group and DM+CAD group(P>0.05).5,Serum ferritin level correlated positively with TC(r=0.42, P<0.05), LDL(r= 0.43,P<0.05), FPG(r=0.37,P<0.05) and have statistical significance(P<0.05). There was no correlation between the concentration of serum ferritin and BMI,HbA1C,FIN,HDL,TG,age and blood pressure.Conclusions:1,Serum ferritin level increased gradually with the development of diabetes, that indicated patients with T2DM have oxidative stress, and this kind of change throughout the whole progress of diabetes. And was aggravated gradually with the development of diabetic angiopathy.2,Serum ferritin level increased by the time of IGT, which suggested serum ferritin was a factor of insulin resistance, it may lead up to insulin resistance.3,Serum ferritin level correlated positively with TC, LDL, FPG, and there was no correlation between serum ferritin level and BMI,HbA1C,FIN,HDL,TG,age and blood pressure, which reflect dyslipidemia may affect serum ferritin.4,Iron load plays an important role in the progress of atherosclerosis. Serum ferritin could be an index to evaluate the risk factor of atheroclerosis as a reliable index of iron load.
