Objective:This study was designed by measuring the septic patients H-FABP, observe the changes in septic Patients, further evaluation of whether it can be used as monitoring of septic patients an effective indicator of early myocardial injury and to explore with the clinical prognosis relevance, explore sepsis by the TNF-a, CRP and other inflammatory factors involved in the inflammatory response to myocardial injury.Methods:1. For the Second Hospital of Tianjin Medical University intensive care unit in December 2008-2,009 years in November screening of patients admitted to select the line 2001 SCCM/ESICM/ACCP/ATS/SIS diagnostic criteria for sepsis, sepsis 66 cases of patients in the control group for health checkup or non-cardiac diseases and infectious diseases, a total of 30 cases of non-septic patients. Recorded in Patients with general information:name, sex, age, Primary disease, length of stay, the Prognosis, as assessed by indicators based on the worst day of acute Physiology and chronic health evaluationⅡ(acute Physiology and chronic health evaluation, APACHEⅡ). Within the past 7 days of acute myocardial infarction, external chest compression cardiopulmonary resuscitation, defibrillation, direct current cardioversion, chest trauma, thoracic surgery, myocarditis, chronic renal insufficiency and muscular disease Patients are the exception.2. Detection Method:Heart-type fatty acid binding protein were detected by the Shenzhen Kang Sheng Bao Bio-Provided CardioDetect (?) med colloidal gold method kit for the qualitative detection. Tumor necrosis factor-a were detected by Purchased from Tianjin Boya Biological Company's ELISA kit kit for the quantitative detection. The remaining biochemical indicators such as cTnI and CRP by my hospital room and Immunology biochemical tests assist in the completion.3. Statistical Methods:KS test for normally distributed quantitative data±s said that the non-normal distribution of quantitative data with the median, maximum, minimum, said the rate indicated by qualitative data. Normal distribution of the qualitative data using t test between the two groups, three groups were compared using analysis of variance, non-normal distribution of the qualitative data with the rank sum test, quantitative information such as the rate used to compare the chi-square test, correlation analysis using Pearson-related, and the Possible risk factors, multivariate Logistic regression analysis, the selected Pairs of meaningful indicators of Prognosis, all data are SPSS16.0 statistical software to complete, P<0.05 for the difference statistically significant Results:The percent of H-FABP Positive and level of CRP, TNF-αare higher in severe sepsis group and the dead group;The level of CRP, TNF-αare higher in H-FABP Positive group which have organ dysfunction and worse Prognostic, the difference has statistic significance(P<0.05).Conclusions:1. Heart-type fatty acid binding protein can be used as myocardial injury monitoring indicators in patients with sepsis.Patients with sepsis, especially in patients with severe sepsis tend to have higher incidence of myocardial injury.2.Inflammatory reaction may occur in myocardial injury in patients with sepsis and has a role.3. with the severity of the disease related to a certain extent, H-FABP are not the prognosis independent risk facor of patients with sepsis.
