Objective:To investigate the effect of pulmonary rehabilitation program to the patients with chronic obstructive pulmonary disease, such as the lung function, arterial partial pressure of oxygen, partial pressure of carbon dioxide, sleep quality,six minute walking test distance, quality of life, daily exercise tolerance, the classification of fatigue, and so on. To investigate whether the continuing care can improve the patients's dyspnea degree. To discuss the proper model of continuing care to the patients with COPD.Methods:102 cases of inpatient with COPD were investigated involving history,symptoms,physical examination, blood gas analysis,six minutes walking distance, body mass index after pulmonary function test was performed on the day when they were discharged from the hospital, one month later, and three month later.. Each patient was required to fill out the St George's respiratory questionnaire, Pittsburgh Sleep Quality Index, social support rating scale, Fatigue Scale-14, activities of daily living. The control group will get normal respiratory care only, on the other hand, the case group will be given breath exercise training, health education, oxygen therapy guideline,et al. we went to the clients home to collect their disease correlated information in one month, three month time, at the same time, to emphasize the continuing care to the case groups.Results:There were no differences between the case group and the control group on the blood gas analysis, lung function, six minutes walking distance, quality of life, sleep, daily exercise activity, and the classification of fatigue on the discharge day (P>0.05). After pulmonary rehabilitation, patients' lung function, physical and mental fatigue has no improvement. The body mass index, arterial partial pressure of oxygen, partial pressure of carbon dioxide, six minute walking test distance, quality of life, daily exercise tolerance of the two groups were found to be of statistics significance between the discharge home day,one month and three month since out of hospital (P<0.05), furthermore, the above index was also of statistics significance between the first and the third time point (P< 0.05).The exacerbation frequency of disease in the case group was significantly less than the control group.Conclusions:The continuing care model of pulmonary rehabilitation program has no effect in improvement patients' lung function,physical and mental fatigue. However, it can increase their weight, meliorate gas exchange, enhance exercise tolerance, improve their quality of life, also can make them sleep well, do more activities all by themselves, at the same time,decrease their dependence on the families. This model can cut down the exacerbation frequency of disease.
