Objective To explore feasibility of production of devascularization of de-splanchnic circulation of stomach in canine models and investigate the effect of de-splanchnic circulation of stomach (DSCS) on portal hemodynamics in prehepatic portal hypertensive canine models and morphological changes of gastric mucosa and esophagus.Methods Twelve prehepatic portal hypertensive canine models were produced by narrowing the portal vein and using silk line chronic embolization. After splenectomy, DSCS was performed by preserving pericardial blood vessels, esophageal varices and vagus nerve and by amputating all gastric veins that flow into the splanchic circulation including gastric coronary vein at the root. Gastric venous blood reflowed into the systemic circulation by pericardial blood vessels and esophageal varices. The canine models were divided into three groups randomly, splenectomy group, pericardial devascularization group and DSCS group. The portal venous pressure (PVP) and esophageal variceal pressure (EVP) was measured respectively by puncturing superior mesenteric vein and pericardial varices preoperatively, postoperatively and 3 weeks postoperatively. The hepatic function was also tested preoperatively and 3 weeks postoperatively, including alanine aminotransferase(ALT), albumin(ALB) and globumin(G). The specimen of esophagus and stomach were proceed for H-E staining 3 weeks postoperatively, and then the pathological changes of esophagus and injury extent of gastric mucosa were observed.Results 1. All the canine models survived for three weeks.2. The PVP was significantly decreased from (23.2±3.1)cmH2O to (17.3±1.5)cmH2O after DSCS (P<0.05). The PVP was significantly decreased from (20.8±2.9) cmH2O to (16.1±1.2)cmH2O after DSCS (P<0.05). There was no obvious difference among three groups in the hepatic function (P>0.05). 3. There was no obvious difference among three groups in the hepatic function, including alanine aminotransferase(ALT), albumin(ALB) and globumin(G) (P>0.05).4. The number of esophageal varices and the injury extent of gastric mucosal in DSCS group was reduced more obviously than that in pericardial devascularization group.Conclusion DSCS can significantly decrease the PVP and EVP and improve the histologic changes of portal hypertensive gastropathy..
