| ObjectiveTo explore risk factors of types 2 diabetes in patients with Nonalcoholic fatty liver disease, and stduy the association of fatty liver with blood glucose.MethodsPatients with Nonalcoholic fatty liver disease were selected from the medical ward, Fujian provincial hospital from Jan.2004 to Dec.2009.88 cases of Nonalcoholic fatty liver disease with type 2 diabetes were enrolled into study group.The control group were made of 61 cases of simple Nonalcoholic fatty liver disease.Comparing the variables bwtween the two groups,including sex,age,BMI,smoke,the family history of type 2 diabetes mellitus,blood pressure,fasting plasma glucose,fasting insulin,2h insulin,glyoosylated hemoglobin,tri-glycerids,cholesterin,low density lipoprotein,high density lipoprotein, APOA1,ALT,AST,GGT,ALP and so on.All the factors were studied by statistical method of univariate and multivariate analysis.Then we can explore the risk factors of types 2 diabetes in patients with Nonalcoholic fatty liver disease.Besides,based on the ultrasonic diagnosis of fatty liver,we can stduy the association of fatty liver and blood glucose.ResultsRisk factors of types 2 diabetes in patients with Naonalcoholic fatty liver disease included sex,age,BMI,smoke,the family history of type 2 diabetes mellitus,blood pressure,fasting plasma glucose,fasting insulin,2h insulin,glyoosylated hemoglobin,triglycerids,cholesterin, low densith liporotein,high densith liporotein, APOA1,ALT,AST and so on. Smoke,BMI, fasting plasma glucose,fasting insulin,2h insulin, triglycerids,cholesterin,low densith liporotein,high densith liporotein, APOA1,ALT, the family history of type 2 diabetes mellitus were significantly correlated with risk by univariate analysis.However,the age,sex, blood pressure, ALT,GGT and ALP were not significantly correlated with the risk.The family history of type 2 diabetes mellitus,fasting plasma glucose,fasting insulin,2h insulin, triglycerids, APOA1 were identified as the most important risk factors by multivariate analysis. In the study group,We can notice that with the increase of blood gluse and glyoosylated hemoglobin,the level of severity increased as well.The severtiy was positive correlated to FBG and GHB.Conclusions1. The family history of type 2 diabetes mellitus,fasting plasma glucose,fasting insulin, triglycerids are the risk factors, while the 2h insulin is the protective factor.2. So the early intervention and treatment of fatty liver and the improvement of insulin resistance can delay the occurrence of diabetes in patients with NAFLD.3. With the increase of blood gluse and glyoosylated hemoglobin,the level of severity increased as well.4. For the patients with diabetes,controlling the blood glucose actively can improve the fatty liver.5. TO study the risk factors of NAFLD patients with diabetes, and then intervent and cure the diabetes can provide important clinical significance. |
