Metabolism,inflammation and malnutrition were commonly found in hemodialysis patients.Hyperlipidemia,inflammation and malnutrition are correlative with atherosclerosis.Adiponectin (Adiponectin, ADPN) has the effect of anti-inflammatory and anti-atherogenic,but at present there are few studies in the relationship between dyslipidemia, inflammation and malnutrition in MHD patients. In this paper we preliminary discussed the relationship between dyslipidemia, inflammation and malnutrition in MHD patients.Objective: The current study was performed to determine the levels of ADPN in patients with hemodialysis,and to examine the possible correlations with plasma lipids,CRP,ALB,BMI,renal function and diabetes mellitus,and to discuss the possibility of ADPN estimate the risks of the occurrence of cardiovascular disase in MHDpatients.Methods:Serum ADPN in 30 hemodialysis patients and 30 health subjects was tested by enzyme linked immunosorbent assay(ELISA);Cr,CRP,ALB,plasma lipids and blood giucose etc was measured.Collected clinical date of every group,such as age,weight,body mass index etc.The correlations between APN and factors was analyzed.Results:1.The serum LDL,TG of hemodialysis patients were higher than normal controls,the serum HDL of hemodialysis patients was lower than normal controls (P<0.01).The serum CRP of hemodialysis patients was higher than normal controls (P<0.01).The serum ALB,Hb of hemodialysis patients were lower than normal controls(P<0.01).2.ADPN levels were higer in cases of hemodialysis patients than in normal controls(P<0.01),The ADPN levels had negative correlation with BMI,ALB,TG,CRP,the correlation coefficient were -0.422,-0.609,-0.492,-0.691(P<0.05), and positive correlation with HDL,Scr, the correlation coefficient were 0.428, 0.407(P<0.05)。Hemodialysis patients with serum CRP concentrations>or=5mg/L were characterized by a lower serum adiponectin concentration than those with the CRP<5mg/L(P<0.01).Conclusions:1. Metabolism,inflammation and malnutrition were commonly found in hemodialysis patients.2. ADPN levels were higer in cases of hemodialysis patients than in normal controls,which were closely related to lipid metabolism, micro-inflammatory state and nutritional status.3. The reasons of high expression of adiponectin levels in hemodialysis patients were on one hand the renal dysfunction and the decline of GFR, on the other hand, a variety of inflammatory cytokines and chemokines resulting in the outcomes after severely impaired renal function.They were adaptative changes of metabolic disorders. The high expression of adiponectin levels was a compensatory response.4. Although the adiponectin levels in dialysis patients were high expression,the abnormalities in hemodialysis patients were irreversible changes caused by multiple factors,adiponectin could not fundamentally chang this situation. |
