Irrational use of drugs in the world today is a very serious problem, more than 70% of chinese population are in rural areas, and the primary doctors account for the most central position in various influence factors of using drug safely and rationally in rural areas.April 2009, the new medical reform program issued, which focused on the majority of primary areas, such as rural areas and communities, and especially strengthened the construction of basic-level medical teams.The objective of this paper is to investigate the situation of common diseases, drugs, traditional Chinese medicine using, adverse drug reactions, primary doctors'status, and the practical needs of rational and safe medication of Guangdong rural areas, as well as research the training mode of rational use of drugs for primary doctors through questionnaires, under the policy background of the new medical reform.The main results of this study are as follows:1. Primary doctors in rural areas were male and in 31~ 50 age group mostly, most had received technical secondary education, with more than 11-20 years work experience.2. Medical diseases were the most common diseases, such as hypertension, influenza, bronchitis, colds, upper respiratory tract infection, etc.3. The kinds of chemicals commonly used were antipyretic analgesic anti-inflammatory drugs, digestive system drugs, respiratory drugs, and the antibiotics commonly used were cephradine, amoxicillin, penicillin sodium, cefalexin, ceftriaxone. 4. The number of traditional Chinese physicians and Chinese medical pharmacists was less.Traditional Chinese medicine injections were mainly used for cough, asthma, expectorant, and the majority was the Qingkailing injection.5. Anti-microbial drugs were prone to ADR, the common ADRs were skin and skin appendage harm, gastrointestinal reactions and anaphylactic shock.ADRs which were more difficult to be dealed with were kidney damage, liver damage and bone marrow suppression. The main reasons for ADR occurrence were inappropriate drug combination, drug quality problems and improper drug selection6. The majority of respondents had participated in rational medication training, and the most times a year was four times.Time restriction was the most important constraints for training, training content was the primary concern factor for the doctors in rural areas. The primary doctors in rural areas expect local concentrated study, the training lecturers were from the top three hospital physicians, pharmacists, and the training was at public expense.7. Training mode research contained the exploration of fund mode and the application of remote expert service platform. Fund operation involved fund planning, expert solicitation and subject start. The main functions of service platform were consulting service and online training.
