The Study Of Vascular Damage And Relation Research In Patients With Metabolic Syndrome

Objective Study the changes of carotid artery intima-media thickness(IMT)and pulse wave velocity(PWV) of metabolic syndrome(MS) patients, and the relevance between them;Discuss the main factors that effect IMT and PWV;Study the target organ damage of MS,and the relevance between target organ damage and IMT,PWV. Methods Choose 90 objects, including 30 healthy objects as mactched group,30 objects with MS,30object with hypertension and metabolic syndrome(HMS).1.Through measuring IMT and PWV to analysis the changes and the relevance of them.2.The MS patients divided into IMT thickness group and IMT normal group(PWV faster group and PWV normal group).Then compare the difference of every dangerous factor between IMT thickness group and IMT normal group(PWV faster group and PWV normal group). Measure the dangerous factors that may affect IMT and PWV,then analysis the main factors which have large affection of them.3. Do the echocardiography, after measuring left ventricular internal dimension (LVD), interventricular septum thickness(IVS) and left ventricular posterior wall thickness(LVPW), then calculate the left ventricular mass(LVW) and left ventricular posterior wall thickness(LVMI).Do fundus examination on all the objects, then evaluate and confirm the period of the fundus atherosclerosis;Use rapid immune turbidimetry to measure concentration of morning microalbuminuria(MA) and calculate the creatinine clearance (CCr).Compare the difference of heart structure,fundus atherosclerosis and kidney function between three groups.Then study the relevance of heart structure,fundus atherosclerosis,kidney function and IMT,PWV. All data will be used for statistical analysis of SPSS11.5. Results 1.IMT and PWV of MS group and HMS group are higher than matched group(P <0.05),and the HMS group higher than the MS group;Also we find that IMT and PWV have a positive correlation. 2.The age, systolic blood pressure(SBP), diastolic blood pressure(DBP), the years of smoke and postprandial blood glucose(PBG) of IMT thickness group are higher than the normal group(P <0.05).High density lipoprotein-cholesterol(HDL-C) is lower than the normal group(P <0.05). The main factors that affect the IMT are Age, SBP and smoke. And the age,abdominal circumference, SBP,PBG and the years of smoke are higher than the normol group(P <0.05),HDL-C is lower than the normol group(P <0.05).The main factors that affect the PWV are age and SBP.3.The LVD,IVS,LVPW,LVW,LVMI,the fundus atherosclerosis and MA of the MS group and HMS group are higher than the matched group(P <0.05),and Ccr is lower than the matched group.And LVD,LVW,LVMI,fundus atherosclerosis and MA of HMS group are higher than MS group,Ccr is lower than MS group.Also we got a conclusion that there is a positive correlation between the LVD,IVS,LVPW,LVW,LVMI and IMT,PWV.They have the most correlation to LVMI. It also have a positive correlation between fundus atherosclerosis,MA and IMT,PWV.There is a negative correlation between Ccr and IMT,PWV. Conclusions 1.The MS patients' IMT and elastic artery change a lot, then the IMT and Elastic artery have a positive correlation, with IMT thickening, PWV become fast.2.The main factors that affect the IMT are Age, SBP and smoke. So we should control the dangerous factors to prevent complications. 3.Also we find that the MS patients will cause the target organ damage ,such as heart,brain and kidney .The target organ damage and IMT,PWV maybe have the parallel development.
