Objects:To investigate the therapeutic effects of the Chinese medical herb , Xingnaojing injection, on the serum and cerebrospinal fluid levels of inflammatory cytokine sFkn in patients with traumatic brain injury (TBI) and study their clinical meaning.Methods:The 82 patients with TBI were randomly divided into two groups ,the Xingnaojing injection treatment group ( n = 42) which Xingnaojing injection was injected into every patient's vein besides routine treatment ;and the control group ( n = 40) which the only routine treatment was applied to. The patients were divided into three groups(poor recovery and good recovery)according to Glasgow Outcome Scales(GOS)after 3 months.The CSF and serum sample would be collected on the 1st,3rd, and 10th day after head injury. sFkn in CSF and serum in each person was tested by enzymelinked immunoadsordent assay.Result:1 The serum levels of sFkn in the patients with head injury on the 1st and 3rd day after injury stepped up. The serum levels of sFkn in the patients of Xingnaojing injection treatment group on the 3rd and 10th day after injury were significantly lower than that in the patients of control group (P<0.05/ P<0.01).2 The CSF levels of sFkn in the patients with head injury on the 3rd and 10th day after injury stepped down. The CSF levels of sFkn of the patients of Xingnaojing injection treatment group in 10th day after injury were significantly lower than that in in 1st day after injury (P<0.01).The CSF levels of sFkn in the patients of Xingnaojing injection treatment group on the 3rd and 10th day after injury were significantly lower than that in the patients of control group (P<0.01).3 In the good recovery group ,the serum and CSF levels of sFkn on the 10th day after difeerent treatment were decreased than on the 1st and 3rd day; while on the 10th day, the serum and CSF levels of sFkn in treatment group were lower apparently than the control group. The Xingnaojing injection may decrease the serum and CSF levels of sFkn, inhibit Inflammatory reaction, lessen Inflammatory injury of craniocerebral trauma.4 In the poor recovery group after difeerent treatment,the serum levels of sFkn on the 10th day were increased than on the 1st day; the CSF levels of sFkn on the 10th day were decreased than on the 1st day ,but remaining high levels .while on the 1st,3rd and 10th day, the serum and CSF levels of sFkn in treatment group were no significant deference than the control group,P>0.05. The Xingnaojing injection had not show inhibition to levels of sFkn In the poor recovery group . High levels of sFkn may have to do with poor recovery tightly.Conclusion:1,Compared with the 1st or 3rd , sFractalkine in serum and CSF of all patients with TBI were significantly decreased after trauma especially at 10th day after the treatment. Xingnaojing injection could decrease the levels of sFractalkine significantly , compared with that after the treatment of the control group. Xingnaojing injection can inhibit releasing of inflammatory cytokines and decrease the CSF and serum levels of sFkn .2,The Xingnaojing injection may decrease the serum and CSF levels of sFkn in the good recovery group , inhibit the degree of pantosomatous and partial inflammatory reaction, lessen the incidence of hyperpyrexia the second inflammatory injury of craniocerebral trauma.But in the poor recovery group, catastaltic role of the Xingnaojing injection to the serum and CSF levels of sFkn were not discovered.Inflammatory cytokines play an important role in secondary brain injury following TBI. sFkn is a sensitive early biochemical neuromarker. The CSF levels of sFkn were closely related to the severity of head injury outcome. The levels of inflammatory cytokines sFkn were important parameter for estimating the severity of head injury and predicting outcome in patients with head injury.
