The Related Factor Analysis Of Lateral Lymph Node Metastasis In Mesolow And Low Site Rectal Cancer

Objective:To investigate the related factors of the lateral lymph node metastasis in mesolow and low site rectal cancer patients, in order to facilitate constitute rational surgical treatment. Methods:Sorted of 203 cases patients'clinical history underwent surgery in Gastrointestinal Surgery Cancer Hospital attached to Xinjiang Medical University, from June,2004 to June,2010.Using SPSS 17.0 statistical software,and the X2 test and Logistic regression analysis, analysis the relationship between the lateral lymph node metastasis and mesolow and low site rectal cancer,and the risk factors of mesolow and low site rectal cancer. ResuIts:All cases lateral lymph node metastasis rate was 18.2%, the above of lymph node metastasis rate was 48.3%, above lymph node metastasis patients with lateral lymph node metastasis was 33.7%, the lateral lymph node metastasis associated with above lymph node metastasis was 89.2%. Gender, ethnicity, surgery type, and the lesion site were not correlated with mesolow and low site rectal cancer (P> 0.05). Univariate correlation analysis showed that age, family history, tumor length, gross type of tumor, histological type, differentiation, depth of invasion, violation of circumference, serum CEA, tumor thrombus and lymph node metastasis in the above lymph node metastasis associated with rectal cancer metastasis(P<0.05). Multivariate analysis showed that age, histological type, depth of invasion, gross type, differentiation degree and above lymph node metastasis were independent risk factors of mesolow and low site rectal cancer (P<0.05). In the age groups:the difference of the rate of lateral lymph node metastasis between< 40 years and 41 to 60 years,> 60 years in the mesolow and low site rectal cancer patients was significantly (P<0.017); invasion depth the difference of the rate of lateral lymph node metastasis between T4 and T3, T2 was significantly (P<0.017); general type, tumor type, ulceration, infiltrative difference between the three groups was statistically significant (P<0.017), pairwise comparisons among groups,there was significant difference (P<0.017). Differentiation in the difference of the rate of lateral lymph node metastasis between poorly differentiated and moderately differentiated, well differentiated in the lateral rectal cancer patients with lymph node metastasis was significantly (P<0.017). Conclusion:Age, histological type, depth of invasion, gross type, differentiation degree, above lymph node metastasis were the independent risk factors of lateral rectal cancer. It has great significance to study the related factors of lateral lymph node metastasis in mesolow and low site rectal cancer,and was propitious to establish a reasonable surgical treatment.
