| [Objective]To compare the effects of dynamic condylar screw and locking compression plate on treatment of type C fracture of the supracondylar and intercondylar femur,and Comparison of different types of fractures in the application of the two effects.[Methods]From 2002 to 2009,56 patients with femoral fracture were treated operatively,and dynamic condylar screw(DCS) and locking compression plate were used.33patients with C type of distal femure fractures treated by internal fixation with LCP which involved 23 male and 5 females with the age from 20 to75.C2 type in 27 cases and C3 type in 5cases. 5 cases of open fractures and 28 cases of closed fractures. 3 cases of anterior cruciate ligament injury, 5 cases of meniscal tear, no damage of blood vessels and nerve. 23patients with C type of distal femure fractures treated by internal fixation with DCS which involved 18male and 5 females with the age from18 to79.C2 type in 18 cases and C3 type in 5cases. 2 cases of open fractures and 21 cases of closed fractures. 2 cases of anterior cruciate ligament injury, 3 cases of meniscal tear, no damage of blood vessels and nerve. Treatment were compared plaster external fixation, fracture healing, knee function.there was no statistical difference about clinical datas among two groops.Qualiative data were processed with X~2 test and quantitative data with t test.[Results]1) The average healing time was (7.86±1.30) months in the DCS group and (7.71±1.06) months in the LCP group. there were no significantly difference in the healing time between the two group with t test(t=0.47,p>0.05).2) The average motion degree of the knee was 80.40±4.26°in the DCS group and 90.38±10.59°months in the LCP group. there were significant difference in the motion degree of the knee between the two group with t test(t=11.04,p<0.05) .3) External fixation:the rate of using external fixation of LCP and DCS was 18.1%,95.6%.There was statistical difference(p<0.05).4) According to Neer's criteria,there were significant difference in the good functional rehabilitation rate between the two group withχ~2 test(χ~2=8.4,p<0.05).[Conclusion]1. Dynamic condylar screw for fractures of C1 and C2.2. LCP treat various types of fractures, but the treatment C2, C3 type and patients with more severe osteoporosis of its advantages.3. Internal fixation with locking compression plate can render patients with type C fraction of supracondylar and intercondylar femur better functional recovery of the knee and greater motion degree of the knee than dynamic condylar screw. |
