Objective:To evaluate the efficacy and safety of two different laparoscopic surgical approach for adrenal gland tumor.Method:Clinical date on 42 patients who underwent transperitoneoscopic adrenal tumorectomy were retrospectively compared with those on 73 who underwent retroperitoneoscopic adrenal tumorectomy for adrenal gland tumors, including patient demographic date, preoperative indexes, intraoperative indexes and the postoperative clinical outcomes.Results:In the transperitoneal group, the mean age was 43.2y,20 cases in the left side and 22 cases in the right side, the tumor size from 1.0cm to 6.4cm, the mean diameter was 2.1cm. In the retroperitoneal group, the mean age was 44.6y,34 cases in the left side and 39 cases in the right side, the tumor size from 1.2cm to 6.5cm, the mean diameter was 1.9cm. There were no significant difference in age, sex, location, tumor size in the two groups. The operative time in the transperitoneal group was 64 min, and 43 min in retroperitoneal groups,the difference between the two groups was significant (P<0.05). The intraoperative estimated blood loss in the retroperitonea group showed a significant advantage (60±28ml VS 115±39ml). In addition, the time to first diet and in the transperitoneal group was 23 hours, and 19 hours in the retroperitoneal group, no difference was observed. The postoperative hospital stay in the retroperitoneal group were shorter than that of transperitoneal group (6.8±1.2d vs 8.4±1.6d, P<0.05). The complication rate was similar in the two groups (P>0.05)Conclusion:The retroperitoneal approach have a significant advantage, which can become the first choice for the treatment of adrenal gland tumor.
