Objective: To explore the clinical case characteristic of Influenza A (H1N1), so that we could discover and diagnose Influenza A (H1N1) early and avoid misdiagnosis, thereby cure this disease, improve prognosis, elevate life quality and provide important evidence for reducing infectivity.Materials and methods: 35 patients of Influenza A (H1N1) diagnosed in our hospital between Match 2009 and Match 2010 were analyzed retrospectively. The age, gender, sensitive factor, main symptom, assistant examine, clinical diagnose, treatment and prognosis of 35 Influenza A (H1N1) were Summarized, in order that we could diagnose, cure and prevent this disease at early state. Compared Influenza A (H1N1) with SARS in 2003 and avian influenza between 2003 and 2006, we could summarize their respective characteristic.Results: There were 20 males and 15 females in 35 cases, with a mean age of 40.22±23.43. male vs female is 4 : 3, age between 22months and 81 years. According to the new age criterion, we divided them into five groups. There were 9 males between 0-44 years old, 5 between 45-59 years old, 4 between 60-74 years old, 2 between 75-89 years old, above 90 years old had zero; There were 10 females between 0-44 years old, 2 between 45-59 years old, 3 between 60-74 years old, above 75 years old had zero. 35 cases had more or less health problems, 4 had maternity problem, 5 had blood system problem, 5 had incretion system problem, 5 were postoperative, 3 had neural problem, 2 were COPD patients, 11 were healthy with 2 obesity people, the percent of cases with basical disease was 68%, the percent of residual healthy cases was 32%. The main clinical symptom of 35 case manifested fever and cough, fever 88.6%, cough 68.6%, some case behaved sputum and dyspnea, part of cases were followed with chest tight, breathed hard, angina and flowed tears, few patients also suffered twitch, emptysis, gasp, swirl and headache. There were also some abnormal in laboratory, the percent of white blood cell that below normal level was 42.9%, normal level was 51.4%, above normal level was 5.7%; anoxemia 74.3% with 23.1% using breathing machine; LDH increased 45.7%, normal 25.7%, 28.6% patients had not done this examination. 34.3% suffers were short of potass. In imaging, 42.9% patients manifested similar to interstitial pneumonia, take on ground glass-like change, diffuse reticular, fibrin strip shape, honeycomb shape shadow with dark edge; the same count manifested similar to pneumonia, present group piece, spot slice, strip shape, node shape shadow; the normal was 14.2%. All of 35 cases were given Oseltamivir orally, 75mg twice a day, some cases combined with bacteria infection were given antibiotics according to experience, when the sputum cultivation result came back, we could adjust antibiotics, severe cases were given Glucocorticoid in vein, breath machine assisted aerate and sustain theatment. 3 cases of 35 died, 2 cases abandoned treatment, the rest straightened up and leave hospital.Conclusion: (1) most patients of Influenza A (H1N1) were young and middle- aged people, the distinction between male and female was not evident. (2) Influenza A (H1N1) had its amiable factor, such as basical disease, pregnancy, obesity and so on. (3) The main symptom of Influenza A (H1N1) presented fever and cough, clinical symptom had no special character. (4) all of Influenza A (H1N1) patients were given Oseltamivir orally, assisted antibiotics and small dose Glucocorticoid, combined with sustain treatment, large part of patients had good curative effect. (5) Compared Influenza A (H1N1) with SARS in 2003 and avian influenza between 2003 and 2006 on epidemiology, clinical symptom, laboratory, treatment and prevent measure, they had no distinction, however, Influenza A (H1N1) had a lower mortality rate and harm. |
