Objective:To investigate the clinical features, diagnostic and therapeutic methods of pancreatic neuroendocrine carcinoma.Methods:The clinical data of 21 patients with pancreatic neuroendocrine carcinoma confirmed by pathological examination were analyzed retrospectively and related literatures were disccused.Results:The 21 patients, including 12 males and 9 females,had an age range of 27-72 years.9 patients received surgical treatment:5 segmental pancreatectomy,11 segmental pancreatectomy and splenectomy,1 extended pancreaticoduodenetomy+the reconstruction of portal vein+splenectomy+excision of liver metastasis,1 segmental pancreatectomy+splenectomy+excision of stomach and colon metastasis.1 of them received chemical therapy after surgical treament. One of the two patients who didn't received surgical treatment received chemical therapy; another received symptomatic treatment received because of jaundice. All cases were confirmed to have neuroendocrine carcinomas by post-operative pathological examination. Lymph node metastasis occured in 3 case and liver metastasis in 3 cases. Colon, stomach, spleen and portal vein invasion was found in 1,1,1 and 1 patient, respectively. Immunohistochemical studies of these patients showed high in CgA,Syn. One of the 21 patients died of multiple organ failure because of multiple metastasis after the operation. other patients were improved.Conclusion:Most pancreatic neuroendocrine carcinoma patients show no chracateristic symptoms. Preoperative diferential diagnosis from other pancreatic tumors is very difficult and confirmed diagnosis needs post-operative pathological examination. Prognosis of pancreatic neuroendocrine carcinoma is better than that of pancreatic is aenocacinomas. |
