| Objective:1 To study the impact factors of patients with non-small cell lung cancer (NSCLC)treated with target therapy; 2 In many factors, study on age, gender, physical status score, family history, histology, tumor stage, number of chemotherapy, smoking index and EGFR expression, which factors have positive effect on the patients with targeted therapy; 3 Draw a linear prediction equation, and forecast which NSCLC patients gain the best treatment by target therapy. Methods:1 Using the Cluster sampling method, Collate information on all patients which was diagnosed with non-small cell lung cancer and treated by targeted therapy in Cancer Hospital of yunnan from January 2009 to January 2010. Besides, all datas was inputted by Epidata.2 Study on factors including age, gender, Physical status score, family history, histology, tumor stage, number of chemotherapy, smoking index and EGFR expression by Logistic regression model, which factors have effect on the NSCLC patients treated by target therapy. Results:1 The total number of NSCLC patients treated by target therapy were 73. Result showed that 31 patients got partial response (42.5%),12 patients had no changed (16.5%), and 30 patients deteriorated (41%).2 After analyzing by Logistic regression analysis of single factor, the impact factors of NSCLC patients treated with targeted therapy were:2.1 Gender:the effection of female patients who treated with targeted therapy was more effective than male patients. And the response rate was 3.96 times as the deterioration rate. 2.2 Histology:The efficacy of targeted therapy in patients with adenocarcinoma was more effective than the patients non-adenocarcinoma. As well as the response rate was 7.69 times as the deterioration rate.2.3 smoking index:The efficacy of targeted therapy in patients who hardly smoke (smoking index=100) was more effective than the patients with long-term smoking(smoking index=400). The response rate was 4.55 times as the deterioration rate.2.4 EGFR expression:EGFR-positive patients with targeted therapy was more effective than negative patients, and the response rate was 5.26 times as the deterioration rate.2.5 number of chemotherapy:the patients with less 4 times chemotherapy were more effective than others, and the response rate was 2..68 times as the later.3 Using the logistic regression analysis to analyze the above five factors by form of combinations, the impact factors of NSCLC patients treated with targeted therapy in multiple factors were:3.1 EGFR expression:In the control patient's sex, smoking index and histology, EGFR-positive patients with targeted therapy was more effective than negative patients; In the control other'affecting factors, EGFR-positive patients with targeted therapy was still more effective than negative patients, and the response rate was 7.69 times as the deterioration rate.3.2 number of chemotherapy:In the control patient's sex, smoking index and histology, the patients with less chemotherapy were more effective than others; In the control other'affecting factors, the patients with less chemotherapy were still more effective than others, and the response rate was 2..66 times as the later.Conclusion:1 The efficacy of NSCLC targeted therapy was related to the patients'gender, histology, number of chemotherapy, smoking index, EGFR expression.2 The patients'EGFR expression and the number of chemotherapy were closely related to the efficacy of targeted therapy, which could be selected as a clinical indicators of targeted therapy among the appropriate population. |
