purpose, necessity, situation and other aspects, combining with Chinese conditions, through theoretical study and spot survey, to understand its current situation of third-level hospitals in Nanjing. To realize and generalize the factors which obstruct the advance of pharmaceutical care, and the influence which come from it so as to change the phenomenon of unreasonable drug application. Looking for the proper way of third-level hospitals'outpatient pharmaceutical care, to ensure drug can be used more safely, effectively, economically, appropriately while the health and medical system keeps changing.Methods: There are 19 third-level hospitals in Nanjing. According to the different classification, 5 hospitals were selected by random sampling. 818 subjects were got, including doctors, outpatients and the whole pharmacists in the above selected hospitals. All data were input into computer with the software EpiData, and analysed with the SPSS 13.0.Results and conclusions:(1) Patients do not have a clear cognition on pharmacist and outpatient pharmaceutical care. In the research, 24.4% patients do not know the occupation of pharmacist, and 79.7% patients have not heard of pharmaceutical care. Most of the outpatients are affirmed the importance of pharmacy, but do not satisfy the pharmacists'service, and more than half of those hope medical institution can provide guidance and counseling about how to take medicine . (2) Clinic physicians do not sufficiently understand the pharmaceutical care, and do not quite agree the influence of outpatient pharmaceutical care in improving the medical quality, increasing awareness of rational drug use and other aspects. Unlike patients, doctors need pharmacists to provide comprehensive, timely, accurate drug information. 98.9% doctors welcome or accepting outpatient pharmaceutical care which provided by pharmacists, but they have different views about whether their knowledge and abilities can meet the service requirements, only 16.5% think they can.(3) Pharmacists as service provider already have a certain degree of cognition on the concept, but they still have some deviations about its content. Comparing the data that from doctors and pharmacists, we can find that they have the same comprehension and evaluation of its overall goals, contents and functions, however, there are some differences in concrete numerical value. 36.5% pharmacists think their knowledge and abilities can meet the service requirements,but only 16.5% doctors have the same idea.Policy recommendation: To establish and improve relevant laws, regulations and industry standards system; to strengthen policy to support pharmaceutical care; to reform Pharmacy Education; to change ideas and pay attention to the outpatient pharmaceutical care; to improve the professional quality of pharmacist; to build appropriate service model and evaluation system.
