| Objective: To investigate the relationship between clara cell secretory proteins and pulmonary function in the perioperation of cardic valve replacement.Methods: 20 consecutive hospitalized patients from April to August 2010 were selected at the first affiliate hospital of Fujian Medical University.The patients suffered rheumatic mitral valve disease and their mitral valve needed to be replaced under the condition of cardiopulmonary bypass(CPB). Blood samples were obtained from radial artery,arterial gas analysis and hematokrit were performed at time of anesthesia induced before CPB(T0),2,6,12,24,48,72h after CPB(T1~T6).Serum CC16 were tested by enzyme-linked immunosorbent assay(ELISA) .The value of oxygenation index(OI,PaO2/FiO2), avolar-arterial oxygen pressure difference(D(A-a)O2),respiratory index(RI,D(A-a)O2/ PaO2)were calculated respectively.Results: Compared with the baseline T0,the mean value of OI and serum CC16 were obviously lower from T2 to T6 (P<0.05),the mean value of RI was higher from T2 to T4 (P<0.05) , the mean value of D(A-a)O2 were lower at time of T5 and T6.By Pearson correlation analysis ,the OI and serum CC16 had a significately positive correlation, the correlation coefficient was 0.696(p=0.000),the RI,D(A-a)O2 and serum CC16 had negative correlations ,the correlation coefficients were -0.664(p=0.000),-0.683(p=0.000) respectively.Conclusion:In the term of 72hours after CPB, Clara cell secretory proteins were participate in the changing of pulmonary function,and played an important role in lung injury caused by CPB, it maybe possitive to the lung,lung injury mainly happened between12and 24hours after CPB. |
