Aime:Percutaneous coronary intervention (PCI) is the most common and important mode of coronary revascularization in monder society. Because tirofiban can prevent the binding of fibrinogen on the surface of the platelet.it frequently is used in PCI. The aim of the this article is present a pooled analysis of randomized clinical trials comparing high-dose tirofiban vs.placebo in patients undergoing PCI for coronary heart disease, with specific emphasis on the role of benefit and safety of high-dose tirofiban regimen.Methods:Basing on the principles and methods of Cochranc systematic reviews.we searched PubMed(1966 to jauanry 2011),the Cochrane Library(2010,5 issue). Embase(1974 to October 2010),Medline China lournal Fulltext Database(1990 to jauanry 2011), Journal of the American College of Cardiology (1990 to jauanry 2011). European Heart Journal (1990 to jauanry 2011). Circulation (1990 to jauanry 2011) Seven randomized trials comparing tirofiban vs. placebo are searched. We evaluate the quality of the RCTs according to the Cochrane Handbook for Systematic Reviews of Interventions Version5.0. The meta analysis use the Cochrane Collaboration's software RevMan 5.0Results:Seven randomized trials comparing high-dose tirofiban vs. placebo were identified,3636 patients being involvied. Meta-analysis shows that high-dose tirofiban can decrease the occurrence of re-myocardial infarction (OR 0.53. (95%CI0.36 to 0.78).P 0.001 and 30-day death (OR 0.63.95%(CI 0.39 to 1.02).p 0.06) obviously.Alough The incidence of minor bleedings is more in patients with high-dose tirofiban OR 1.39.95%(Cl 0.96to 2.01),p=0.08),it can be control soon and readily.Conclusion:Comparing with placebo. the high-dose tirofiban can decrease the occurrence of re-myocardial infarction and 30-day death obviously.The minor bleedings caused by high-dose tirofiban can not result death. We can get higher benefit from high-dose tirofiban...
