objective to compare anticoagulation safty and efficacy of three different methods in continous venous-venous hemofiltration.Methods patients undergoing CVVH were enrolled in this study. They were assigned to 3 groups:groups A using regional citrate anticoagulation(RCA) with citrate infusion rate of 27-31mmol/h. Group B using routing dose of low molecular weight heparin (LMWH).Group C using combination of RCA and low dose of LMWH. The loading dose were 40IU/kg and 200IU/kg in group B and C, while the maintain dose were 4IU/kg/h and 2IU/kg/h for each. Filter span life and hemorrahage were observed, we compared blood gap and blood routing test before and after CVVH.Result A total of 76 filters were used, the median lifetime of filter were 14.4±1.8h,19.0±5.0h,23.8±4.6h in each group. Two major bleeding occurred in group B. Conclusion 1.The combination of RCA and low dose LMWH can prolong filter span life with fewer bleeding episodes.2.Monitoring is important in anticoagulation of CWH.第5页...
