Objective: This study aims to survey the status of nurse staffing in Shanghai community family practice and analyze the problem; Construct the conceptual model of nursing human resources configuration of community family practice service in Shanghai, ensure the facts that influences nurse staffing, and provide the theoretical foundation to the government to make policies.Method: The researcher designed the questionnaires to take a survey of the status of nurse staffing in Shanghai community family practice service and analyzed the problem; ensured the facts that influenced nurse staffing on qualitative research, constructed the conceptual model of nursing human resources configuration of community family practice service in Shanghai based on Entity-Relation method. Compared our policy with abroad on nurse staffing in community family practice service through literature review and proposed recommendations.Result: (1) Status of nurse staffing: normally it was the nurses in the community medical service center or sanitation service sites that provided family practice nursing care; when the nurses were on holiday, there would be someone who worked in other departments or in other positions work for them; more than half nurses hadn't received any training on community care or general nursing in the past five years; the nurses'education structure: bachelor's degree or above accounted for 5.96% of the total, tertiary education accounted for 42.13%, secondary education accounted for 51.23% and those who had no education accounted for 0.68%; Age structure: those who were younger than 30 years accounted for 24.93%, 31-40 years old accounted for 44.09%, 41-50 years old accounted for 20.51%, those who were older than 50 years accounted for 10.47%; Title structure: Nursing titles accounted for 28.77% , Nurse Practitioner accounted for 48.76% and Nurses-in-charge accounted for 22.47%. (2) Results of survey of implementers of the nursing items:①Lack of operational capacity of home care nurses;②I nsufficient number of nurses;③There was security risk in family practice nursing care. (3)Results of job satisfaction survey: the overall satisfaction score was 3.3±0.42 points, living in the general level of satisfaction; the separate dimensions, interpersonal satisfaction score was 4.36±0.48 points, living in the satisfactory level; intensity of work satisfaction score was 3.59±0.67 points, the satisfaction score was 3.58±0.65 points, working conditions satisfaction score was 3.53±0.58 points, living in the less satisfactory level; job prospects satisfaction score was 3.38±0.82 points, social status satisfaction score was 3.14±0.75 points, work stress satisfaction score was 3.03±0.69 points, living in the general level of satisfaction; satisfaction score of occupational risk was 2.61±0.55 points, living in not satisfied level in comparison; relative reward satisfaction score was 2.49±0.75 points, living in not satisfied level. (4) Got the four main elements that influenced nurse staffing in Shanghai community family practice based on qualitative research: the medical policies that the government made,the way that the medical colleges trained the students,the factors of the nurses,the requirements of the patients to nursing. Construct the conceptual model by Entity-Relation method and gave qualitative description of the relevance and role of the four elements. Proposed recommendations to the policies:①Make standard for accessing and training systems;②Improve the structure of the nurses;③Make security system on family practice.Conclusion: Shanghai municipal government departments should attach importance to family practice nursing care, allocate the nurses reasonably, improve its configuration structure, make strict accessing standard and training system of the nurses, make security system on family practice so as to provide safe and high quality of nursing care to the aging society, and promote social development and harmony of our society.
