The Clinical Study Of The Effect On Knee Stability Made By Isokinetic Training In The Case Of Knee Osteoarthritis

Objective:Research on the range of motion,muscle power,stability of the knee about patients suffered from osteoarthritis of the knee through 23 cases of patients with osteoarthritis of the knee were tested and trained by constant strength training apparatus on CMSi。Method:All patients were elected from the rehabilitation department of Chain-Japan Union Hospital of Jilin University, who accepted conservated treatment because of suffered from knee osteoarthritis from December 2008 to December 2010。Inclusion criteria:①patients diagnosed stageⅡor stageⅢaccording to K/L for radiological diagnosis;②During treatment, the patients only cured by physical therapy (ultrashort wave, magnets, etc.),not including internal drugs (such as hormonal, nonsteroidal anti-inflammatory drug), joint lumen injection and surgical intervention;③can take care of oneself during daily life;④patients who do not have high blood pressure,diabetes,heart diease and other important organ failure and so on;⑤Patients who were diagnosed OAK by using the criteria formulateed by the American rheumatism in 1995。There are 23 casas, involving 26 knee joints,11 patients cured from December 2009 to December 2010 were set to the controlled group,12 patients December cured from 2008 to December 2009 were set to the uncontrolled group。In the controlled group there are one male, others are female; aging from 35 years old to 53 years old, the average is 45.32 years old; two involved two knees respectively。In the uncontrolled group there are one male, others are female; aging from 36 years old to 55 years old, the average is 46.83 years old; one involved two knees。All patients have no obvious traumatic history。Patients of two groups have different degrees of knee pain, muscle power drops and function limited obstacles。The range of motion of the knee:the controlled group is between 80°to 95°,the average is 85.5°,the uncontrolied group is between 78°to 92°, the average is 84.7°; The pain degree score:the controlled group is between 3 to 8, the average is 5.3°, the uncontrolied group is between 2 to 8, the average is 4.9; Dysfunction in scoring:the controlled group is between 5 to 13, the average is 9.1, the uncontrolied group is between 5 to 13, the average is 8.8°; The controlled group:we test the range of motion,the muscle power by CSMi isokinetic muscle testing system, and know the degre of dysfunction by using visual analogue scale and Lequesne index before treatment。Constant muscle test:select test speed for 60°/s,90°s,120°/s,150°/s, first measure health side, then the other side, if involved both idiomatic side first。During testing, patients were take seat, hip and trunk into angle 110°, joint axis Angle 110,fixed the waist, chest, leg which can produce compensatory sites. Dynamic arm is fixed on 3cm before the calf condyle with nylon chalaza, The testing order is firstly slowly then quickly。Before each test, patients did JiLiang knee flexion as preparation activities for 3 times and then do their best to stretch knees。During slowly testing flexion and stretch knees for five times, while quickly testing for 20 times, Each test interval is 10s。Constant strength training:every time did passive activity in CSMi system with angular velocity 10°/s, on patients with joint activities limited stop 5s, increase the range of motion progressively with no pain or mild pain, During training the angular velocity/s were 150°/s,120°/s,90°/s,60°/s,90°/s,120°/s,150°/s, each speed did 10 times, each time rest for 20s, firstly did one time, then gradually increased to two or three groups and each group interval 5min,1 times/d,40min/times,6 times per week,10 times for one period of treatment, three periods were done。The uncontrolled group:we test the range of motion,the muscle power by CSMi isokinetic muscle testing system, and know the degre of dysfunction by using visual analogue scale and Lequesne index before treatment。Then training through regular exercise therapy, specificly were:patients take seat, therapists are on the side of the patients, whose hand are on the thigh and the ankle, using the forces of the body above applied to put in the hands of ankle, ask patients extensor the knee and the power is changed when the strength of the patient is changed, try their best to maintain a certain speed, the same methods were tolded during exercise of knee flexion, each group did 10 times, all for 10 groups and each group interval 20s。Each treatment time is 40min,6 times per week,10 times for one period of treatment, total is three treatment training。Both groups were testde the range of motion,the muscle power by CSMi isokinetic muscle testing system, and we know the degre of dysfunction by using visual analogue scale and Lequesne index before treatment。Results:After treatment, all patients have different levels of the test indicators improved, but the experimental group than in the control group improvement was large and statistically significant。The peak torque, range of motion, VAS and Lequesne index of the isokinetic muscle training group are improved compared with training before, which indicating that isokinetic muscle strength training system play an important role in enhancing muscle power and joint stability。Conclusion:1. The isokinetic strength training instrument have reliable effection to enhance knee stability。2,With the guideline of the idea of modern rehabilitation, the effectiveness of isokinetic exercise is significantly higher than the traditional strength training on the patients with osteoarthritis of the knee...
