| Objective:To analyze the epidemiology,clinical characteristics,pathological features and its relevance of 517 cases with IgA nephropathy,to find out their epidemiological distribution characteristics and the factors contributing to prognosis,to provide a basis for determining the pathogenetic conditions comprehensively, early detection, effectivly treatment and prognosis.Methods:A retrospective analysis of the epidemiology ,the clinical,pathological and laboratory indicators of 517 cases of IgAN patients confirmed by renal biopsy examination in our hospital from March 4, 2002 to August 31, 2010. I analyze the following aspects.Firstly,the incidence of all ages.Seconndly,the relationship among 24-hour urine protein,renal function and level blood pressure.Tirdly,the relationship among renal function,clinical indicator, and pathological grade.The last,the relationship between pathological grade and clinical indicator.Results:1 General information: the number of primary IgA nephropathy accounts for 21.45% of the total number of renal biopsy examination cases; 268 male cases and 249 female cases,the ratio of male to female was 1.076:1,the incidence age from 10 years to 70 years,and the average age was 33.3±11.83 years, the average course of disease was 21.57±46.85 months ;the apparen peak incidence age was 20 to 40 years old. 328 urban cases and 289 rural cases,the ratio of urban to rural was 1.74:1.2,Pre-onset manifestations:obviously pre-onset infections were 217 cases,accounting for 41.97%. Respiratory tract infections were 107 cases,accounting for 20.7%.Gastrointestinal tract infections were 10 cases,accounting for 1.93%.Urinary tract infections were 20 cases,accounting for 3.87%.Tonsillitis total were 74cases, accounting for 14.13% of the 517cases and 69.16% of respiratory tract infections. Tonsillitis maked up of the most number of respiratory tract infections.Compared with gastrointestinal tract infections and urinary tract infections , there was a significant difference(p<0.01).Gross hematuria was apt to apper after the pre-onset infection, accounting for 41.49% of the incidence of gross hematuria in our research. In the pre-onset infections, respiratory tract infections accounted for 19.15% and tonsillitis 21.28%. .3,The incidence of the age: the incidence of gross hematuria was higher in the young..The incidence of hypertension,remal failure and hypercholesterolemia inereased by aging.4,The relationship among proteinuria,renal failure and blood pressure: The incidence of renal failure inereased by the level of the proteinuria,but no significant effect in hypertension.5,The relationship between renal failure and other clinical indexes:There was significant effect between hypertension and the normal blood pressure group,between hypertriglyceridemia and the normal triglyceridemia group,between hypercholesterolemia and the normal cholesterolemia group.6,The relationship between pathological grade and other clinical indexes:517 cases of IgA nephropathy in patients with Lee's pathological grades up to grade 3,totel 179 cases,accounting for 34.6%.The incidence of Lee's pathological grade 3 was highest both in male and female.There was no significant difference in the incidence of Lee's pathological grade by gender. There was significant difference in the incidence of Lee's pathological grade between hypertension and the normal blood pressure group, between hypertriglyceridemia and the normal triglyceridemia group,between hypercholesterolemia and the normal cholesterolemia group.The proportion of IgG+IgM+C3 immunofluorescence ingredients was increased by aggravating of the Lee's pathological grade.Conclusion:1,517 cases of IgA nephropathy in patients with onset between the ages of 20 to 39 in the largest proportion,with onset of infection up to the incentive,especially respiratory infections. Gross hematuria was apt to apper after the pre-onset infections.2,There was a certain relationship among hypertension,renal function,hyperlipidemia and the age in the 517 cases of IgA nephropathy.3,There was a certain relationship among the incidence of renal failure and IgA nephropathy who accompanied by hypertension,hypertriglyceridemia or hypercholesterolemia.4,The incidence of Lee's pathological grade 3 and IgG+IgM+C3immunofluorescence ingredients was highest in the 517 cases of IgA nephropathy. The proportion of IgG+IgM+C3 immunofluorescence ingredients was increased by aggravating of the Lee's pathological grade. 5,There was a certain relationship among the Lee's pathological grade and IgA nephropathy who accompanied by hypertension,hypertriglyceridemia anemia or renal failure. |
