Objective:This experiment establish rabbit ears vein blood clot flap, and using high doses of restructuring model respectively leech grain leeches grain low-dose restructuring leeches grain injection saline in local injection, Research restructuring leeches element induce venous blood clot flap bFGF expression, promote micro vascular hyperplasia, so as to improve the survival rate of venous blood clot flap, further discussing restructuring leeches element in the venous blood clot flap field application prospect. Methods:Health ordinary big-eared white rabbit, whether male or female,72, weight (2000±or 200g), randomly divided into three groups:2 treatment group (group A:high dose restructuring leeches grain treatment group, group B:low dose restructuring leeches element treatment group) and a control group (group C:injected saline groups), Each group of twenty-four, press 40 mg/kg weight,2% sodium pentobarbital, left ear margin intravenous anesthesia, disinfection and pave sterile towels after anesthesia. General observation:postoperative day 1 3 groups flap distal all appear different degree of organization swelling blood clot, Blank group is more apparent in blood clot degree than in the high-dose leeches group and and low dose restructuring leeches grain group signific,show dark purple, high doses of leeches grain group and low doses leeches grain group with no obvious boundary of congestion belt,blank group with well-defined boundary of congestion belt. Postoperative 3,7 days, high dose leeches grain group and low doses leeches grain group flap distal congestion swelling gradually decline until basic abreaction, healed well.Blank group flap distal and sea.journal color black, qualitative hard, local hair, nocapillaries reaction, tend to necrosis.postoperative 7 days restructuring leeches grain treatment group and control group of survive area percentagel respectively (94.64±1.90)%and (94.64±1.90)%, are higher than saline control group (78.86±1.08)%, significant difference (p< 0.01).High doses of restructuring leeches element and low dose restructuring leeches grain treatment group flap survival rate quite, difference was no statistically significant (p< 0.05).2. Microvascular density (MVD) [1]:Take each group 1,3,5,7 days pathological, In the cytoplasm of yellow dye with around other organizations have obvious bounds of endothelial cells or endothelial cells strings, regardless of what lumen, count for a blood vessels. First on at low magnification (by 100) choose dyeing endothelial cells most areas.,then at high magnification (HP, x 400) next count the vascular number of this one area, record every vascular number at high magnification vision,choose the biggest three numerical averages as MVD. Day 1 each group did not see obvious capillary is generated, article 3,5,7 days of high dose leeches grain group and low doses leeches grain group of capillary number/HP separately for (8.5+0.9) and (5.6+0.7), (11.6±0.8) and (8.5±1.0), (12.0±0.5) and (8.9±1.0), All are higher than blank group (3.5+0.8), (3.0+0.7) and (2.3+ 0.6), it difference all have statistical significance.3. BFGF content detection (pg/ml):Postoperative 1,3,5,7 days,high doses of leeches grain group and low doses leeches grain group bFGF content respectively are (47.32±2.04)and (49.54±2.15),(69.12±1.68)and(54.76±1.52),(82.70±0.61)and(60.11±0.72), (84.48±0.82) and (64.55±0.63),its corresponding blank group are (49.15±1.42), (50.06±1.45), (48.10±0.24) and (50.32±0.51).Article 3,5,7 days, among high dose leeches grain group and low doses leeches grain group and control group, the diffrences have statistically significant (P< 0.01), a\article 5,7 days, between high dose leeches grain group and low doses leeches grain groups the differenceswas statistically significant (P< 0.05).Each group compared each were no statistically significant difference (P> 0.05).4. BFGF MVD and BFGFexpress:High doses of restructuring leeches grain group and low doses leeches grain group and control group in 1,3,5,7 days MVD and BFGFexpression level was significantly positive correlation (r= 0.368, P< 0.01), article 3,5,7 days, high dose restructuring leeches grain group and low dose restruturing leeches grain group MVD and bFGF expression leve wa significantly positive correlation (r= 0.240, P< 0.05). Conclusion:This experiment initially proved that local use restructuring leeches element can induce bFGF expression, promote microvascula hyperplasia,improve flap congestion state,and then inprove venous blood clot flap survival rate.
