Medical Service Efficiency Evaluation Of The Township Health Centers In Guangxi

Objective: To establish the efficiency evaluation index system which is adapt to the Township Health Center, comprehensive and objective evaluation of the efficiency level of Township Health Centers(THCS) in Guangxi, Combined with the status of THCS in in Guangxi analysis the possible problems. For the administrative departments of health to provide a theoretical basis for health policy .To maximize use of limited health resources, for the people to provide safe, effective, convenient and affordable medical and health services. Materials and Methods: The data of this study were 2009 financial statements of the Guangxi Health Department. Included in the analysis of a total of 1,193 township hospitals, which are 257 hospital health centers, general hospitals 936. The data included fundamental state of the THCS , Hospitals operating conditions,Resources,Income and expenditure,Carry out the work situation and other business .Etc.The main methods used in this study are as follows:Literature method: A lot of research paper available in English and Chinese were referred to understand the Progress in this field. And initial screening the efficiency evaluating indexs . Cluster analysis:Based on the literature method,Combined with information obtained and the efficiency evaluating indexes that Can reflect the overall situation of THCS, using clustering index , eventually filter out into the DEA input and output indicators.Data envelopment analysis: By this method to evaluate the overall effectiveness of THCS in Guangxi, pure technical efficiency, scale efficiency and returns to scale status.Result:(一)The basic situation of THCS: The total area of the business space was 1,894,457 square meters, The average operational area was 1,587.98 square meters. The overall utilization rate of hospital beds was 75.81%, the lowest was 6.48%, the highest was 186.03%, Guangxi agricultural population per thousand total number of beds in rural hospitals was 0.94. THCS overall debt ratio was 26.30%, the highest was 79.64%, which was greater than 30% debt ratio were 449. The average income of THCS was 3,047,850 yuan in 2009, the 430,770 yuan from of financial assistance, accounted for the average total income of 14.13%, the average hospital business income accounted for 82.86% of total revenue, drug income which accounted for 46.41% Hospital Revenue. The average total expenditure was 2,833,020 yuan, which 562,370 yuan for business expenses other, accounted for 19.85% of average total expenditure. 47,755 people serving staff, Health workers accounted for 76.81%, Anti-insured account for 10.60%, Per thousand the number of agricultural population was 1.06 health workers.(二) Indicators established and DEA analysis :With Literature method,Cluster analysis,Typical indicator selection, Finally, includee 10 input indicators: Financial assistance income,Operating expenses,Business houses,The average per employee responsible for outpatient and emergency visits,The average per employee responsible for hospital bed days,The average charge for each level of outpatient and emergency visits,The average per bed day charges,The average cost of hospital medicine,The proportion of total revenue income from drugs,Service workers per capita salary income;6 output indicators:Business income,The number of clinics were,Number of discharged patients,Occupancy rate,The average hospital stay was discharged,Bed turnover times. The overall effective DEA THCS were 325 (27.24%), Overall non-DEA efficient THCS increasing returns to scale were 772(64.71%). Decreasing Returns to Scale were 95(7.96%). Strong and effective health care services of THCS more efficient than its effective and non-effective weak THCS.Conclusion:Infrastructure in THCS in Guangxi as a whole was weak; Widespread phenomenon of different degrees of liability; Constitute a source of funds and the use of unreasonable; Personnel constitute not scientific; DEA analysis shows that the whole THCS in Guangxi is relatively inefficient, In which a large proportion of increasing returns to scale, prompted the need to increase financial investment, expand the scale of the THCS.
