| The study is part of the national natural science foundation project "the mechanism study of depression with insomnia treated by Chinese non-pharmacological therapy" (the project number:30772845). It was instructed by my supervisors Dr.Liu Yanjiao and Dr.Wang WeidongObjective:To explore the personality characteristics of TCM five-pattern, polysomnography characteristics of patients with depression, as well as the relationship between them.Method:The study adopted literature study and cross-sectional survey methods. It was carried out under strict regulation of state natural science foundation. Literatures in and abroad of relevant field were referred to under the instruction of tutors. Based on this, we started the study which was assumed reasonable by experts.According to the included criterion, patients are enrolled from psychology department and sleep medicine clinic, China academy of Chinese medical science Guang'An Men Hospital and the project the national natural science foundation project "the mechanism study of depression with insomnia treated by Chinese non-pharmacological therapy". The number of patients is 88. Five-Pattern Personality Inventory and relevant scale data of patients were collected. And the data were compared to national norms; 30 healthy people were enrolled as control in polysomnography study. All controls and patients were examined by polysomnography The data were analyzed; the correlation was analyzed between characteristics of five-pattern personality and polysomnography data. All the data was analyzed by statistical software SPSS 13.0.Result:1 Five-pattern personality characteristics of depressed patientsThe original score of Taiyang and Shaoyang dimensions of patiets are significantly less than national norms (P<0.01);the score of Taiyin dimension is significantly higher than national norms (P<0.01); the score of Shaoyin dimensions is not different from national norms (P>0.05); the score of Taiyin dimensions are positively correlated with score of MADRS, depression and anxiety factor of MMPI(P<0.05或0.01). However, there is no significant correlation between scores of Taiyin dimension and SAS as well as SDS (P>0.05); the analysis of standardized T score indicates the same result.2 Polysomnography characteristics of depressed patientsSleep model:Prolonged sleep latency, advanced REM sleep, increased number of wake and wakeful time, decreased SWS can be seen in depressed patients.Sleep procedure:All parameters of sleep procedure are significantly different from controls':prolonged sleep latency, increased wake times, prolonged wakeful time, increased ASR (P<0.01).Sleep architecture:except REM sleep time, all parameters of sleep archeture are significantly different from controls':increased S1%and S2%, decreased SWS% (P<0.01)REM sleep:obvious REM sleep latency decrease, increased REM sleep percent, decreased number of REM sleep, increased REM activity, intensity and density can be seen in depressed patients (P<0.01)The subjective and objective sleep quality assessment:From the contrast analysis of sleep latency and sleep efficiency, sbjective assessment of sleep quality is significantly different from objective assessment (P<0.01)The correlation between polysomnography parameters changes and depression degree:the correlation between depression scales and items of REM sleep is not significant; SDS score and SL positively correlated (P<0.05); MADRS score and S1, S1%positively correlated (P<0.05)3 The correlation between five pattern personality and polysomnography parameters:The score of Taiyin dimension is positively correlated with sleep latency and REM density (P<0.01);the score of Shaoyin dimension is positively correlated with S2 and S2%(P<0.05); The score of Yin-yang balanced dimension is positively correlated with sleep efficiency, but negatively correlate with number of wakefulness and S1%; the score of Taiyang and Shaoyang dimensions has no correlation with any polysomnography parameters.Conclusion1 The five-pattern personality characteristics of depressed patients:score of Taiyin dimension is higher while scores of Taiyang and Shaoyang dimensions is lower; the score of Taiyin dimension of depressed patients is positively correlated with other depression scale, which indicates that the score of Taiyin dimension can reflect the degree of depression.2 The change of polysomnography indexes reveals that sleep mode changes, disrupted sleep architecture, abnormal REM sleep in depressed people. They are manifested by prolonged sleep latency, increased wake times, prolonged wakeful time, increased ASR, increased S1%and S2%, decreased SWS%, increased REM ratio, shortened REM latency, increased REM activity, REM density, REM intensity; the subjective assessment of sleep quality is significantly different from objective assessment; The more severe of depression, the more difficult in falling asleep and keeping sleep deep. However, items of REM sleep no significant correlation with depressive degree.3 Higher score of Taiyin and Shaoyin dimension has negative influence on sleep quality, such as prolong sleep latency; high score of Yin-yang balanced dimension has good effect on sleep, which indicates that peaceful mind is important for sound sleep.4 In conclusion, higher score of Taiyin dimension may suggest depression; polysomnography of depressed people has typical changes; the five-pattern personality of depressed people can influence their sleep quality. |
