Expression Of Reck, Her-2/neu, MMP-2 In Carcinoma Of Cervix Uteri And Cervical Intraepithelial Neoplasia And Its Biologic Significance

Objective To research into the expression of RECK,Her-2/neu and MMP-2 in squamous carcinoma of the cervix(SCC)and cervical intraepithelial neoplasia (CIN), explore their associations with the carcinogenesis and development of SCC,and evaluate their clinical significances.Methods Sp-immunohistochemistry was used to measure the expression of RECK, Her-2/neu and MMP-2 in 45 cases of CIN, 55 cases of SCC and another 20 cases of normal cervical tissus selected from paraffin wax-embodied specimens. of the 45 cases of CIN, 10 were CINⅠ, 13 were CINⅡ,22 were CINⅢ. 31 were G1.19 were G2,5 were G3 of the 55 cases SCC according to pathologic grading and 30 were stageⅠ,19 were stageⅡ,6 were stageⅢof them according to clinical staging. The testing results were analyzed in combination with related clinical data by using Chi-square test and Spearman correlation analysis.Results 1. MMP-2 was expressed in 3 of 20 cases of normal cervical epithelium and the positive expression rate of it were 15.0%, 24 of 45 cases of CIN and the positive expression rate of it were 53.3%, 3 of 10 cases of CINⅠand the positive expression rate of it were 30.0% , 5 of 13 cases of CINⅡand the positive expression rate of it were 38.5% ,16 of 22 cases of CINⅢand the positive expression rate of it were 72.7%, 40 of 55 cases of SCC and the positive expression rate of it were 72.7%, With CIN progressing while the expression MMP-2 tended to increase, there was signifcant difference among three groups(χ2=6.67,P<0.05). The positive expression rate of MMP-2 in SCC was significantly higher than that in CIN and normal tissues (χ2=20.00,P<0.01). MMP-2 expression was positive correlated with clinical stages, histological grade and muscular invasion (P<0.05).2. RECK was expressed in 13 of 20 cases of normal cervical epithelium and the positive expression rate of it were 65.0%, 14 of 45 cases of CIN and the positive expression rate of it were 31.1%, 7 of 10 cases of CINⅠand the positive expression rate of it were 70.0% , 5 of 13 cases of CINⅡand the positive expression rate of it were 38.5% , 2 of 22 cases of CINⅢand the positive expression rate of it were 9.1%, 3 of 55 cases of SCC and the positive expression rate of it were 4.5%. With CIN progressing while the expression RECK tended to decrease, there was signifcant difference among three groups(χ2=12.36,P<0.01). The positive expression rate of RECK in SCC was significantly lower than that in CIN and normal tissues (χ2=50.88,P<0.01). RECK expression was not correlated with clinical stages, histological grade and muscular invasion (P>0.05).3. Her-2/neu was expressed in 1 of 20 cases of normal cervical epithelium and the positive expression rate of it were 5.0%, 26 of 45 cases of CIN and the positive expression rate of it were 57.8%, 2 of 10 cases of CINⅠand the positive expression rate of it were 20.0%, 7 of 13 cases of CINⅡand the positive expression rate of it were 53.8% ,17 of 22 cases of CINⅢand the positive expression rate of it were 77.3%, 41 of 55 cases of SCC and the positive expression rate of it were 74.5%. With CIN progressing while the expression Her-2/neu tended to increase, there was signifcant difference among three groups(χ2=9.36,P<0.01).but the positive expression rate of Her-2/neu in SCC was significantly higher than that in CIN and normal tissues (χ2=28.92,P<0.01). HER-2/neu expression was positive correlated with clinical stages, histological grade (P<0.05), but not correlated with muscular invasion (P> 0.05) .4. We found that there is the correlation among the expression of RECK, HER-2/neu and MMP-2 during SCC cancerization. The correlation coefficient ranges from -0.811 to 0.345, which shows that they were notably interrelated with one another (P<0.05).Conclusion 1. The expression rate of HER-2/neu and MMP-2 protein was increased ordinally in the normal cervical epithelium, CIN and SCC, which indicating the increased expression of HER-2/neu and MMP-2 may play important roles in the infltration, metastasis and carcinomatous changes of normal cervical epithelium in cervical carcinoma. Her-2/neu expression was positive correlated with clinical stages, histological grade, but not correlated with muscular invasion. MMP-2 expression was positive correlated with clinical stages, histological grade and muscular invasion.2. The expression rate of RECK protein was decreased ordinally in the normal cervical epithelium, CIN and SCC, which indicating the abnormal expression of RECK may play important roles in the infltration, metastasis and carcinomatous changes of normal cervical epithelium in cervical carcinoma. RECK expression was not correlated with clinical stages, histological grade and muscular invasion.3. There is the correlation among the expression of RECK, HER-2/neu and MMP-2 during SCC cancerization, which indicate they may involve in the occurrence and development of cervical lesions and play the important roles in the infltration and metastasis changes of SCC.4. It's possible to prevent cervical carcinoma occurrecing and developing by upregulating expression of RECK and downgulating expression of HER-2/neu and MMP-2 by means of gene therapy and so on.
