Objective To observe the percentage of peripheral blood CD4+CD28-T cells and the change of serum concentrations of human heat shock proteins 60(hHSP60) in patients with acute coronary syndrome(ACS),and in vitro peripheral blood mononuclear cells (PBMCs)in patients with acute coronary syndrome(ACS) were stimulated with recombinant human heat-shock protein 60 (hHSP60), then the percentage of CD4+CD28-T cells were assessed by flow cytometry. To evaluate the role of CD4+CD28-T cells and serum hHSP60 in the occurrence and development of the unstable coronary artery plague and the effect of hHSP60 on the expression level of CD4+CD28-T cell in patients with ACS.Method Three groups of subjects were enrolled:group 1,patients with acute coronary syndrome(ACS,n=36);group2,patients with stable stable angina(SAP,n=32); group3, control group who are subjects with normal coronary artery on angiography (NCA,n=32).All of the subjects of the percentage of peripheral blood CD4+CD28-T cells were assessed by fluorescence-activated cell sorting (FACS) employing labelled antibodies to CD4andCD28,the concentrations of serum hHSP60 were measured using an ELISA technique.Peripheral blood mononuclear cells (PBMCs) were separated by use of Lymphoprep,different doses of hHSP60 (0μg/mL,0.5μg/mL,1μg/mL,5μg/mL) stimulate peripheral blood mononuclear cells, after the cells were incubated overnight for 48h, the percentage of CD4+CD28-T cells were assessed by fluorescence-activated cell sorting (FACS).Results The percentage of peripheral blood CD4+CD28-T cells and the serum concentrations of hHSP60 in ACS group[(6.71±1.10)%],[ln(hHSP60),(5.57±1.10)] were significantly higher than those in SAP group [(2.23±1.03)%],(3.42±1.20)and control group[(2.28±1.09)%],(3.33±1.07) (P<0.01), but, there is no signifinant difference of both of them between SAP group and NCA group (P>0.05). The percentage of peripheral blood CD4+CD28-T cells and the serum hHSP60 levels show positive correlation (P<0.05). There is increasing trend of the expression of CD4+CD28-T cells in different doses of hHSP60 in patients with ACS,but the differences are not statistically significant (P>0.05).however, CD4+CD28-T cells have no response on hHSP60 in SAP group and NCA group.Conclusion (1)Patients with acute coronary syndrome patients suffer immune effector cells and self-antigen disorder,indicating the disorder of immune regulation function be involved in the occurrence and development of unstable plaques.(2)Both CD4+CD28-T cells and serum hHSP60 increased in patients with ACS indicating that they may have a promoting role in the occurrence and development of unstable plaques;(3)The percentage of peripheral blood CD4+CD28-T cells and serum hHSP60 levels show positive correlation, and hHSP60 can induce amplification of CD4+CD28-T cells, indicating that the interation of both CD4+CD28-T cells and hHSP60 may expand the immune and inflammatory response in atherosclerotic plaque and add to plaque instability,leading to the occurrence of acute coronary syndrome(ACS).
