Background:"Acute coronary syndrome(ACS) is a serious harm to people's health as a common disease and frequently-occurring disease,which is caused by an important cause of sudden death".Due to coronary artery stenosis or complete occlusion,ACS will result in myocardial ischemia or necrosis and give rise to corresponding symptoms. Percutaneous coronary intervention therapy(PCI) can carry out coronary revascularization is an effective method of treatment of ACS,which is a minimally invasive surgery. The advantage is that significantly can improve symptoms of myocardial ischemia in patients with coronary heart disease.Additionly cardiac function was improved to varying degrees,increase quality of life and reduce the incidence of cardiovascular events. The disadvantage is that may cause minor myocardial damage (MMD) that can evoke vascular remodeling after no-reflow phenomenon,furthermore commonly used cardiac troponinI(cTnI),myoglobin(Mb), creatine kinase-MB(CK-MB) and other cardiac markers detect the PCI myocardial damage.Ischemia Modified Albumin(IMA) is the emerging field of medicine as a response to a biochemical marker of myocardial ischemia,in comparison with other indicators,which has emerged in the early,very sensitive,negative predictive value of higher benefits,moreover attracting people's attention.Objective:By observing the therapy of percutaneous coronary intervention (PCI) in plasma before and after surgery in patients with ischemia-modified albumin, cardiac troponin and creatine kinase isoenzyme concentration and to explore its value in early diagnosis of coronary heart disease.Methods:Sixty patients with coronary heart disease (CHD) were studied in our hospital between May 2009 and December 2009.Fourty patients with coronary heart disease were treated with PCI and 20 patients diagnostic angiography (controls).During the PCI therapy, blood samples were collected for determining IMA, cTnI and CK-MB before PCI in the 0.5,6,12hours after the operation.Finally 20 cases were diagnosed only by coronary angiography with the control group(CG),also at the same time points corresponding indicators.But those patients who received PCI, were recorded with the place, number of stein insertion and the change of ECG in 24h after PCI.Results:IMA of 34 patients in 40 patients was elevated from the baseline after PCI.Median IMA levels were higher after PCI compared with the baseline (P< 0.05), which were decreased to normal levels in 6 hours.cTnI levels were raised after PCI in 20 of the 40 patients.Levels of CK-MB remained unchanged (P>0.05) during 12hours after PCI.In control group, IMA, cTnl and CK-MB levels remained no change before and after angiography(P >0.05).Conclusion:According to causing light after PCI injuries on myocardium, levels of IMA were quickly increased and correlated with severity of myocardial ischemia and before significant elevations of cTnI.IMA is not only a sensitive marker of myocardial ischemia, but also is an indicator of the severity of myocardial ischemia in during PCI and Ischemia modified albumin is a promising marker of cardiac ischemia.
