Objective: We compared the efficacy, and the main recurrenc, the rate of stone Clearanc, the time of operation, the day of hospitalization, recurrence, postoperative adjuvant therap, the safety of percutaneous nephrolithotomy (PCNL) with ureteroscopic lithotripsy (URL) for the treatment of impacted upper ureteral stones. Methods: A total of 95 patients with large imacted uer ureteral stones, defined as the stones characteristicd ang Ureter condition and stones>0.8 cm in size and adhesions with circum-ureter, the stones located between the ureteropelvic junction and the lower border of the fifth lumbar vertebra and adhesions with circum-ureter, were prospectively Randomized for antegrade (50) or retrograde (45) ureterolithotripsy. The efficacy and the main recurrenc, the time of operation, the day of hospitalization, recurrence, postoperative adjuvant therap, the rate of stone Clearanc at 3days follow-up,1 month follow-up,3month follow-up were analyzed in each group. Results: The stone—free rate at l month follow-up was 97.6% in percuta-neous nephrolithotomy group and 85.1% in ureteroscopic lithotripsy group.The mean degree ofhaematoglobin in PCNL and URSL groups decreased 3.05% and 0.56% respectively. The rates of complica-tions in PCNL and URSL groups were4% (2/50),and 4.4% (2/45),the time of operation was 97.8 minites and 41.1 minites. The mean hospital stay was 20.7days and14.1days. The mean hospital cost was 23283 yuan and 16732yuan. Conchsiom For an impacted, upper ureteral stones, PCNL had a better stone-free rate and few complications which could be the first choice.
