Background The traditional MME (M-mode Echocardiography) Teichholtz method, 2DE (two-dimensional echocardiography) of the Simpson double-plane method are the most common methods to evaluate ventricular systolic function. RT-3DE (real-time three-dimensional echocardiography) can estimate the ventricular volume without any geometric assumptions, and can accurately measure the ventricular volume under a variety of forms.However, the subjects'breath, heart rate, or the movement of body position would impact it, and need to import the collection data off-line to stations to measure and analyze, either can not be assessed online, All of these limit its wide application in clinic. So, it's very necessary to seek a more rapid, simple and accurate method to measure ventricular volume and function in the clinical work. tri-plane imaging using a new technique to acquire, reconstruct and analyse ultrasound image, real-time display the heart chamber's 3 two dimensional surface each with 60°angle in the one cardiac cycle, in the case of a fixed probe.That can scan more sections, enrich observed content and also shorten the examination time, so the clinical application efficiency can be improved. Be able to accurately measure ventricular stroke volume provides a new approach to the clinical that can evaluate ventricular function fastly and easily.Objective To explore the real-time tri-plane echocardiography in left ventricular volume measurement with the M-type and 2DE of the consistency and relevance, and to investigate the feasibility and accuracy of RT-3PE (the real-time tri-plane echocardiography) quantitative determination left ventricular systolic function of congenital cardiopath perioperatively.Instruments and methods GE Vivid7 Dimension-mode color ultrasound diagnostic apparatus equipped with real-time three-dimensional(RT-3D) ultrasound imaging and three-dimensional ultrasound probe (3V) (frequency,1.5~4.0MHz) was used in diagnosis. From June 2008 to January 2010,57 patients (37 males and 20 females), aged 2~12 years (4.94±2.97) with congenital heart disease, of which 19 patients with atrial septal defect,23 patients with ventricular septal defect and 15 patients with patent ductus arteriosus underwent operation.Real-time tri-plane Echocardiography (RT-3PE) was used to measure quantitatively left ventricular end-diastolic volume (LVEDV) and end-systolic volume (LVESV) to calculate left ventricle ejection fraction (EF) and stroke volume (SV) before surgery,5~7 days and one month after surgery. Meanwhile, the functions of left ventricular ware evaluated with other two methods 2DE Simpson and MME to compare with RT-3PE.As control group, the functions of left ventricular in thirty normal subjects were measured by the above mentioned three methods (RT-3PE, 2DE Simpson and MME).Results (1)LVEDV, LVEDS and LVSV measured by three different methods were significantly different in any two methods (P<0.05), of which the values measured by MME were maximum, the values measured by 2DE biplane Simpson were minimum. LVEF showed no significant difference compared with 2DE. The values measured by the three methods were relevant. The values measured by RT-3PE and 2DE biplane Simpson showed good relationship (Normal group r=0.869, CHD group r=0.914, P<0.001), and the values measured by RT-3PE and MME also showed correlation (Normal group r=0.719, CHD group r=0.792, P<0.001).(2) LVEDV, LVESV, LVSV and LVEF measured by RT-3PE in patients with ASD were lower than those in control group (P<0.05). (3) LVEDV, LVESV, LVSV and LVEF measured by RT-3PE 5~7 days after surgery in patients with ASD were higher than those in control group (P<0.05). LVEDV, LVSV and LVEF measured 1 month after surgery were higher than (P<0.05) those measured 5~7 days after surgery, however, LVESV showed no significant difference (P>0.05).(4) LVEDV, LVESV, LVSV and LVEF measured by RT-3PE in patients with VSD/PDA were higher than those in control group (P<0.05).(5) LVEDV, LVESV, LVSV and LVEF measured by RT-3PE after surgery in patients with ASD/PDA were lower than those measured after surgery (P<0.05). LVEDV, LVESV and LVSV measured 1 month after surgery decreased and LVEF increased but showed no significant difference(P>0.05).Conclusion Real-time tri-plane echocardiography can easily and accurately determine left ventricular volume and calculate the left ventricular systolic function. can quickly and accurately determine ventricular volume of congenital cardiopath and found that the changes in ventricular function conditions, it would play a significant role in the clinical diagnosis and postoperative evaluation of efficacy of congenital heart disease, so it has a potential for clinical application.
