| Objective: To study the clinical effect of surgical treatment for diabetic foot with peripheral ateries disease.Methods: From September 2008 to December 2009, the clinical data of 35 patients with diabetic foot were collected in the First Affiliated Hospital of Soochow University.All was teated by only endovascular treatment or endovascular treatment combined open vascular operation .Results: In the 35 patients (43 limbs), 39 limbs were teated successfully. 2 limbs were improved slightly , but go back to the preoperative state after two weeks, and even heavier; 2 limbs failed;at last, the 4 limbs were amputated at different levels, the surgical success rate was 90.7%. During the follow-up, 6 limbs appeared restenosis or occlusion (all were below-knee artery), the ABI between 0 to 0.4, all were teated again by PTA combined indwelling catheter thrombolysis, the symptoms of lower limb ischemia disappeared, ABI achieved 0.7. In the 35 patients (43 limbs) with foot ulcers, 31 patients were cured. Postoperative follow-up has no ulcer recurrence and new ulcers.Conclusions: 1 Percutaneous transluminal angioplasty (PTA) is still the fist choice in the treatment for diabetic foot with peripheral ateries disease. 2 For diabetic foot with multisegmental peripheral ateries, endovascular treatment combined open vascular operation can achieve a better result. 3 The Medium- and Long-Term clinical effect on endovascular treatment combined open vascular operation is also need further observation. 4 Diabetes is a systemic disease, diabetic foot is one of complications. the comprehensive treatment for diabete is very important in the treatment for diabetic foot. |
