Contrasting Analysis On Common Bile Duct Stone Therapy Using Endoscopic Versus Surgical Procedure

Objective:To evaluate the adventage and disadventage of endoseopic therpay and surgical operation by analyses of 66 cases who were caught with common bile duct stone(CBDS).Methods:66 patients of CBDS treated in the First Affiliated Hospital of Dalian Medical University from January 2008 to July 2008 were analyzed. These cases were divided into surgical operation treatment group (S group) and endoscopic treatment group(E group) according to their treatment methods. Through a comparison to see wether the clinieal materials, the rate of treatment and stone clearance, recent complications, first time eating after treatment between two groups had any significant importance, meanwhile, related literatures were reviewed.Results:32 patients were performed by surgical operation treatment,18 cases were male,and 14 cases were female, mean age of 68.4±14.2 years, mean size of stone1.35±0.38cm,mean diameter of common bile duct 1.33±0.42cm,14 cases of single stong,18 cases of multiple stones,34 patients were performed by endoscopic treatment,19 cases were male,and 15 cases were female, mean age of 63.9±9.2 years. mean size of stonel.27±0.24cm,mean diameter of common bile duct 1.24±0.38cm,17 cases of single stong,15 cases of multiple stones, here was no difference between two groups(P＞0.05). Besides CBDS, there are 17 cases has other disease of S group(51.3%), the E group has 24 cases(75.0%), There was same difference between two groups(P＜0.05). The rate of treatment and stone clearance of S group and E group were 100%vs 94.1%, 84.4%vs 81.3%, there were no difference (P＞0.05). The rate of recent complications of S group and E group was 31.3%vs 15.6%.10 patients of S group were associated with complications(31.3%),1 patient died.6 patients of S group suffer from complications(15.6%), there was significant difference between the two group (P＜0.01). The first time eating after treatment of S group and E group was 47±9 hours vs 24±6 hours.84.4%vs 81.3%, there was difference (P＜0.05).Conclusions:The incidence of CBDS between man and woman is no signi-ficantly different,but the morbidity is arising With increasing age; Endoscopic therapy has the same efficiency with surgical operation on CBDS,but the Endoscopic therapy has less ravage and fewer complications, If patient has other disease or suffer from many treatment, the endoscopic therapy is more safety. But, the surgical operation have a priority when patient has a big stone or complex condition on common duct.
