Objective:To analyze the clinical efficacy of OPLL, CSM, and DCS treated by Double-Door Bone Graft Augmetation Plasty by Slitting Cervical SpineMethods:Follow up 58 patients with cervical melopathy including 28 PLL,17 DCS and 13 CSM,who were operated by Double-Door Bone Graft Augmetation Plasty by Slitting Cervical Spine. They had posterior midline incision.The saw guide tube was inserted between the C7 and the T1 towards the head slowly and imported wire saw between the C2 and the C3 gap.Then ligamentum flavum and processus spinosus were slitted from the inside to outside. Processus spinosus was separated longitudinally after drilling on it and embedded into made-up inverted ladder artificial bone. Finally it was fixed .Retrospective analysis(the range from 12 months to 5 years and 7 months after operation. Average time was 3 years and 2 months).All subjects were evaluated by JOA score, improvement rate of Hirabayashi Lie method. JOA score was statisted by test before or after the operation.The clinical effects of Double-DoorBone Graft Augmenation Plasty by Slitting Cervical Spine in treating cervical melopathy was analyzed. In addition the influence of age ,pathogenesis, and POA score before operation was analyzed.Result:JOA score of 58 patients followed up before and after operation were calculated by t test. P< 0.001, showed that therapeutic effect of operation was significant. The rate of RxC cases of Age of patients with postoperative improvement was calculated by X2 test. X2= 62.837, P<0.001, showed that the difference of JOA improvement rate among different age groups had statistically significance. It indicated that therapeutic effect decreased with the increase of age. Comparing preoperative severity of disease with postoperative effect of operation, P<0.001,indicated that JOA improvement rate in groups with different preoperative JOA score was statistically significant. The lower JOA score before operation,the fewer the change of improvement rate. The RxC of pathogenesis of patients with postoperative improvement was calculated by X2 test. P<0.001,showed that the difference of JOA improvement rate was statistically significant among four pathogenesis groups.It demonstrated that the effect was worse with the extension of the course of desease.Conclusion:Double-Door Bone Graft Augmetation Plasty by Slitting Cervical Spine was an effective surgical method to treat OPLL, CSM and DCS.The factors of affecting operation were as follows:1 Age:The older, the worse the effect; 2 Pathogenesis:the longer the duration, the worse the effect; 3 Preoperative JOA score:the lower the preoperative JOA score, the worse the effect of operation.
