Objective:To im,estigate thepostoperative mortality and major com-plications erpancreaticoduodenectomy and its rela—tionship with clinical riskfactors.Methods:The postoperative mortality and major complication inciden-ce of 265 patients underwent pancreaticoduodenectonty between January 2006 and December 2009 in the 401 hospital of Chinese People's Liberation Army.Univariate and nudativariate analysis were used to analyze clinical risk factors.Results:Univariate analysis showed that major risk factors of postoperative mortality and morbidity of the patients were serum bilirubin Ievel(≥171 umol/L). serum album Ievel(<30g/L). operating time (≥6h), postoperative serum hemoglobin level(<90g/L).intra—operative bleeding (≥1000ml).Multivariate analysis using Logistic regression identified four variables as independent factors associated with the occurrence of Pos-tnperative complications of Pancreatoduodenectony serum bilirubin level (≥171 umol/L).serum album Ievel(<30g/L). postoperative serum hemo-globin level(<90g/L). intra—operative bleeding(≥1000ml).Conclusion:Postoperative mortality and morbidity of pancreatoduo denectomy for periampullary carcinoma are closely related to serum bilirubin level.serum album level.postoperative serum hemoglobin level and intra—operative bleeding.
