Objective:Surgical tissue tumor (sarcoma) is a mesenchymal cell origin, with different clinicopathological features and biological behavior of the rare solid tumor heterogeneity. Surgical treatment of soft tissue cancer, including (1) intracapsular rese-ction (2) resection margin (3) wide excision (4) radical resection (5) amputation. But in the past 30 years, surgeons have been the choice of surgical procedures there is contro-versy. Simple cyst excision and local recurrence rate after marginal excision and distant metastasis rate in the high, low survival rate has been gradually abandoned. Sarcoma in patients with distal limb salvage surgery is usually not the amputation, most patients can not accept the reality of amputation, because of its serious impact on quality of life of patients, but did not increase the survival rate of amputation. So reduce the amputation rate, increase the survival rate is the direction of our efforts. Survival and quality of life, amputation rate will be an important measure of treatment based on the level. This article describes Since 1987, the institute carried out on the soft tissue tumor resection cavity and repair after resection of form and function reconstruction of the experience of 20 cases and statistical analysis, review of the literature review study, further study of soft tissue in the surgical treatment of malignant tumors, resection of the advantages of chamber, efficacy and clinical value. Methods:Since 1987, the institute carried out on the soft tissue tumor resection cavity clinical data of 20 patients. Of which 6 cases of fibrosarcoma, leiomyosarcoma,6 cases of malignant fibrous histiocytoma (MFH) 3 cases of skin bulge of three cases of fibrosarcoma, rhabdomyosarcoma, and synovial sarcoma in 2 cases. Tumor size> 5cm in 14 cases,<5cm in 6 patients. Report of 11 cases of poorly differentiated tumor grade, moderately differentiated in 5 cases, well-differentiated in 4 cases, of which 5 were stageâ… ,â…¡of 12 cases,â…¢of 3 cases. All patients were followed up, the postoperative clinical data were retrospectively analyzed. Determine the procedures for the treatment and clinical value.Results:20 patients were treated with resection chamber soft tissue sarcoma patients,19 cases without recurrence (hospital after treatment), the local recurrence rate was 5 percent,20 patients,1 patient was lost to diagnosis in 5 patients when the outside the hospital for the surgery for recurrent cases, by our hospital, two surgeries in 4 patients without local recurrence more than 5 years,1 case (left temporal nerve fiber sarcoma) after the second operation for less than 5 years, lost one case and left temporal 1 case of sarcoma of nerve fibers were considered as not more than 5-year survival, it survived more than 5 years,18 cases in 5 years the production rate of 90%. Limb function is normal. Distant metastasis in 1 case, the left temporal Neurofibrosarcoma hospital after surgery on it 2 years local recurrence, and distant metastasis occurs. Be implemented after the resection cavity, the second follow-up after 2 years no local recurrence and lung metastasis by chemotherapy and traditional Chinese medicine to stabilize. Conclusion:For soft tissue cancer, to control the tumor and preserve the body for the treatment of the core, cavity surgery is the preferred method. Although the implementation of chamber limb salvage resection of local recurrence rate of patients increased slightly, but the amputation, the survival rate of patients with the same. With local radical resection chamber to retain physical therapy and cure by amputation, local recurrence rate is not significantly different. Tendency of local recurrence and tumor type, but not with histological grade and tumor volume are directly related. The most important is that patients preserve limb function and morphology, no amputation quality of life of patients suffering appears. Malignant tumors in soft tissue surgical choice, compartment resection and amputation in the recurrence rate and 5-year retrospective study of survival was no significant difference. Illustrates the many methods of limb salvage surgery in the resection of the advantages of the chamber.
