BackgroundLeukemia is one of ten high incidence cancers in China with about 2.76/10 million incidence rate, which current clinical remission rate comes to 70% to 90% with the development of modern medicine. Even if the patients achieved bone marrow complete remission, there is still considerable proportion relapsing. Thus, how to prevent relapsing to prolong the survival or even cure patients is the current research focus. Minimal residual disease (MRD) is a major factor in relapsing, research about which is the key. At present, the fact that Traditional Chinese medicine treat for MRD is only by personal experience, the current for MRD, clinical words for a more empirical treatment. They mostly treat MRD by syndrome differentiation, such as supporting the healthy energy and strengthening the deficiency, dispelling the toxin or both above. Due to the lack of syndromes derived by clinical epidemiological research as the foundation, it is necessary to carry out clinical epidemiological research about syndromes of TCM.ObjectThis subject initially determines the basic TCM syndrome of MRD by clinical epidemiological statistics, which lay the foundation for TCM treating MRD and further clinical study of standardized therapy for MRD.MethodThis research explains from the review of the literatures and clinical epidemiology study two aspects. Review the literatures about the present situation of the Western and Chinese medicine treating for minimal residual disease of acute leukemia and put forward the problems and focus of the current research. The clinical research collects 180 patients diagnosed as minimal residual disease in acute leukemia from January 2005 to December 2009 in Hospital of Guangdong Province, Guangzhou Military General Hospital hematology ward through clinical epidemiological investigation. According to the results of literature research and referent standard of TCM symptoms, we design and fill in the collection form about TCM symptoms of MRD in acute leukemia. Then use SPSS13.0 software for frequency analysis on TCM symptoms appeared in 180 cases with MRD of acute leukemia to differentiate symptoms according to the diagnostic standard.ResultIn the distribution of TCM symptoms about 180 cases of minimal residual disease in acute leukemia, fatigue is the most frequent among the individual symptoms, accounting for 93.3%, followed by the spontaneous sweating, night sweats, Poor appetite, heavy limbs etc.. The highest frequency of tongue demonstration is pink tongue with white fur, accounting for 25.6%, followed by pink tongue with yellow fur, pale fat indentedbars tongue side with white fur etc.. The most frequent pulse is stringy small pulse, accounting for 23.9%, followed by weak deep pulse, small weak pulse, slippery stringy pulse etc. Qi and Yin deficiency is the main symptom in TCM symptoms, accounting for 29.4%, followed by Qi deficiency, Yin deficiency with stasis and the other eight TCM syndromes. In the fact of asthenia and demonstration syndrome, asthenia and demonstration syndrome is the main symptom, accounting for69.4%, followed by asthenia syndrome, accounting for30.6%, there is no demonstration noun. Qi and Yin deficiency is the main symptom in TCM symptoms after frequency analysis, accounting for 47.2%, followed by damp obstructing and Qi deficiency, deficiency of both Qi and Yin accompanied with stagnation of blood, deficiency of both Qi and Yin accompanied with dampness and stagnation of blood, deficiency of both Qi and blood accompanied with stasis.ConclusionTherapy for minimal residual disease in acute leukemia belongs to "consumptive disease" in TCM, which pathogenesis is the deficiency of healthy energy and vestiges of toxin. deficiency of both Qi and Yin is the main symptom in TCM symptoms, followed by dampness obstructing and Qi deficiency, deficiency of both Qi and Yin accompanied with stagnation of blood, deficiency of both Qi and Yin accompanied with dampness and stagnation of blood, deficiency of both Qi and blood accompanied with stasis After analysis 180 patients diagnosed as minimal residual disease in acute leukemia. Combining disease and syndromes, I suggest vestiges of toxin should be description in the five TCM Syndromes, deficiency of both Qi and Yin with vestiges of toxin in MRD is the commonest. The initial establishment of the basic TCM syndrome conforms to the current understanding of Chinese medicine field. Because the case is limited by the source, are in the south, small sample size, combined with the subjectivity of Chinese dialectical conclusion might have some bias. Still needs to be multi-center clinical epidemiology research before large sample further established the basic TCM syndrome treating MRD. |
