ObjectiveTo detect the levels of matrix metalloprotease (MMP)-1 and matrix metalloproteinase-7 in the serum and the bronchaoalveolar fluid (BALF) of the patients with idiopathic pulmonary fibrosis and sarcoidosis at II phases and to investigate the significance of their changes in the pathogenisis of IPF.MethodsClinical confirmed the cases of patients with IPF were 44, ranged from 46 to 70 years old, averaged (58±9) years old, the cases of patients with sarcoidosis were 20, ranged from 35 to 65 years old, averaged (50±12) years old. Enzyme-linked Immunoabsordent Assay was used to detect the levels of MMP-1 and MMP-7 in the serum and the BALF of patients with IPF and sarcoidosis.Results1. In the serum of patients with IPF, the levels of MMP-1 3.78 (0.14-13.44)μg/L were lower than that of the sarcoidosis 7.79 (4.67-10.68)μg/L (z=-3.53, P<0.01), and the levels of MMP-77.83 (3.57-14.37)μg/L were higher than that of the sarcoidosis 4.04 (0.06-9.94)μg/L (z=-3.84, P<0.01), in the BALF of patients with IPF, the levels of MMP-1 1.09 (0.04-5.14)μg/L were lower than that of the sarcoidosis 2.08 (0.05-4.16)μg/L (z=-1.53, P>0.05), and the levels of MMP-7 3.75 (1.10-9.87)μg/L was higher than that of the sarcoidosis 1.16 (0.02-4.47)μg/L (z=-5.33, P<0.01).2. MMP-7, in the serum of patients with IPF showed stronge negatively correlation with diffusing capacity of carbon monoxide(r=-0.56, P<0.01) and the percentage of neutrophils (r=-0.47, P<0.01), In the BALF showed negetively correlation with diffusing capacity of carbon monoxide (r=-0.31, P<0.05).Conclusions1. In the serum and BALF of patients with IPF, the levels of MMP-1 were lower than that of the sarcoidosis; In the serum and BALF of patients with IPF, the levels of MMP-7 were higher than that of the sarcoidosis, it suggests that MMP-1 may higher in the inflammatory infiltration phases than matrix remodeling period of lung fibrosis; MMP-7 may higher in the matrix remodeling period than in the inflammatory infiltration phases of lung fibrosis.2. MMP-7 may be as an important indicator of the severity of IPF.
