| Objective:To study the effect of dental anxiety (DA) on the blood pressure and heart rate of patients with tooth extraction, and compare the influence of local anesthesia (LA) and general anesthesia (GA) on the blood pressure and heart rate of patients with tooth extraction. Providing psychological therapy and nursing guidance for patients before extraction. The right anesthesia method should be used to improve the safety of extraction for patients.Methods:(1) A modified Dental Anxiety Scale (DAS) was used in 120 outpatients with tooth extraction. ECG was used in the whole process of extraction. Then the correlation between DAS and the changes of heart rate (HR) or blood pressure (BP) was analysed. (2) A Detax-ohmeda anesthesia monitor was used to detect the HR and BP before and after anesthesia in 80 LA patients and 76 GA patients with extraction. Then we compared the effect of the two methods of anesthesia on the HR and BP of patients, and evaluated the clinical significance of the above monitoring indicators.Results:(1) 9.2% of all the patients had DA. Scores were significantly higher in such patients as female, who had low extent of pain endurance and who was in tense mood on that day. The correlation between the score of DAS and HR was insiginificant. while the correlation between the score of DAS and the increases of BP has positive relationship (P<0.05). (2) The HR and BP of patients rose obviously after LA and had statistical significance (P<0.05). Wheareas, The HR and BP of patients maintained stable after GA, and had no statistical significance (P>0.05).Conclusions:The prevalence of DA in patients before tooth extraction is extensive, and many factors are related to DA. It is positive relationship between the increases of BP and the levels of DA, and no significant relationship between the increases of HR and the levels of DA. The result of comparison between LA and GA in patients with tooth extraction showed that the fluctuation of HR and BP were related with the psychological factors. Psychotherapy should be given to the patients, especially for the DA before tooth extraction to reduce DA and to prevent cardiovascular accident. GA is more suitable for DA patients. |
