Objective:Observation of the clinical efficacy and changes in biochemical parameters of plasma exchange (PE) in the treatment of liver failure .Method:Retrospective analysis of a total of 46 patients with liver failure which live in our hospital from March 2004 ~ March 2010 , and according to the different treatments, they were divided into treatment group and control group, the treatment group' patients were treated with comprehensive treatment in conjunction with plasma exchange treatment, and the patients in control groups were treated with comprehensive treatment. we observated the clinical symptoms after treatment changes, including nausea, vomiting, fatigue, abdominal distension and the presence of mental symptoms, and patients were recorded before and after treatment (application of medical treatment or plasma exchange treatment) 1 d ~ 5 d and 10-14 d of liver function, blood coagulation and plasma levels of routine.Results:(1)After regular treatment, the clinical symptoms of the two groups' patients except disturbance of consciousness had improved, the state of consciousness in patients treated with plasma exchange in treatment 1 d ~ 5 d improved, 10 d ~ 14 d increase;(2) After 1 d ~ 5 d of plasma exchange treatment, AST, ALT, ALP,GGT decreased, TBIL,DBIL decreased, GLO decreased, ALB and CHE increased, APTT shortened, PTA increased (P <0.05);(3) After 10 d ~ 14 d of plasma exchange treatment, AST, ALT decreased, ALB, CHE increased, PTA increased (P <0.05);(4) After 10 d ~ 14 d of comprehensive medical treatment, AST, ALT, GGT decreased, PTA increased (P <0.05);(5) In the treatment group and control group, there was no significant difference in length of stay, and in hospital costs, the treatment group were significantly higher the the others (P <0.05);(6) By the formal After treatment, the prognosis of patients with no significant difference (P> 0.05);(7) Before and after treatment by plasma exchange in patients with no significant changes in serum ion levels (P> 0.05);Conclusion:Plasma exchange to a certain extent to improve the clinical symptoms of liver failure,and to liver function,blood coagulation and other biochemical indicators have improved, and no significant effect on serum electrolyte levels, but wishing to effect continuous improvement in the prognosis of patients required a short interval continuity of treatment. |
