| Objective: To analyze MSCT imaging features of Castleman's Disease in two pathological types to improve the diagnosis, provide a reliable basis for clinical diagnosis and treatment.Materials and Methods: Retrospectively analyzed 20 Castleman's disease cases MSCT imaging admitted to our hospital from January 2004 to December 2009, diagnosed, and underwent CT examination, Observed with ages, sexes, clinical presentation and the imaging which included the lesions'location, size, number, boundary, scan density and enhanced features.Results: 12 cases of HV CD (4 males and 8 females), age range was 15 to 59 years old; 6 in 12 found in physical examination, others found in xiphoid under pain, gasp, cough, chest tightness, chest pain, abdominal pain, oral ulcer, neck mass; single lesion in 10 cases, 2 cases of multiple lesions; 5 lesions located at the mediastinum and hilar, 4 lesions located at cervical part, 4 lesions located at retroperitoneal zone and 1 lesions located at armpit. 8 cases of PC CD(3males and 5females), age range was 16 to 73 years old; 1 in 8 found in physical examination, others found in weakness, fever, anemia, cough, gasp, abdominal pain, lower back pain, neck and facial masses; single lesion in 4 cases, 4 cases of multiple lesions; 4 lesions located at cervical part, 4 lesions located at retroperitoneal zone, 3 lesions located at the mediastinum and hilar, 3 lesions located at armpit and 1 lesions located at right facial. 10 in 12 cases of HV CD ware homogeneous soft tissue density, clear boundary, no necrosis, diameter range was 1.5 to 11.8cm, 2 cases ware calcification; 8 in 10 enhanced scan showed even significantly enhancement; others showed inhomogeneous enhancement. 7 in 8 cases of PC CD ware homogeneous soft tissue density, clear boundary, no necrosis; 4 cases showed many parts of swollen lymph nodes; 4 enhanced scan showed mild to moderate heterogeneous enhancement. 9 ceses of this group were misdiagnosed, The rate was 45%.Conclusion: There were significant diffence in age, clinical performance between HV CD and PC CD. Characteristic MSCT imaging features were found in HV CD, combination of age and clinical presentation would be made more precise imaging diagnosis. PC CD lacks the characteristic imaging features, which diagnosis dependented on histopathology. |
