| Object:To investigate medium and long term results of total hip arthroplasty for reconstruction function of hip joint.Method:Fifty-one cases(55 hips)with total hip arthroplasty were reviewed with an average of 83.5 months follow-up (range,60 to 180 months).In this group,the primary diagnosis included avascular neerosis of femoral head (22 hips), femoral neck fracture (26hips),osteoarthritis(2hip),human giant cell tumor of bone(1hip).The clinical results were scored with Harris method,and we got comparison of curative effect between difference of two fixation methods (cement fixation and uncemented fixation) of femoral component in hip arthroplasty. To sum reasons up which caused limp and then analyse major reason. The roentgenographic results were analyzed with Harris and Gruen methods.Results:The average Harris scores increase from preoperative 24.8 points ( range , 4 -88 ) to 76.6 points (range,36-98.6) at follow-up . Roentgenographic results of femoral components showed 19 hips with radiolucent line (3 components with continuous radiolucent line, 16 components with local radiolucent) .Roentgenographic results of acetabular component revealed 6 hips with radioluvent line(3 components with continuous radiolucent line , 3 components with local radiolucent).Conclusion:The study showed that the total hip arthroplasty provides fairly good clinical function and survival in the medium and long term follow up no matter cemented fixation and uncemented fixation. Choosing and loosening of prosthesis was the major cause of pain and limp at follow-up .There were many factors for osteolysis ,the polyethylene wear debdris appears to be the major one. |
