Background:TB (Tuberculosis, TB) is an infectious disease which is seriously harmful to human health. The global TB incidence rate is still growing by about 1% per year. At present, 2 billion people worldwide have been infected with tuberculosis, up to 2 million people with active pulmonary tuberculosis, and 300 thousand people die from tuberculosis. TB has been regarded as a major social problem, the stragedy and and effect evaluation of prevention and treatment of TB have become a hot issue in present public health field. By now, OTS strategy has been recommended by the World Health Organization to the world as the most effective global TB control comprehensive measure which is considered as the most cost-effective measures. However, the implementation of DOTS strategy in various regions is different, each country need to value the strategy with its with their national conditions.Objectives:This study analyze the epidemic of TB in KuanCheng District, Changchun city during 2004-2008 and survey 100 TB patients, inquire the epidemic features of TB distribution in the district after implementing DOTS stragedy and analyze the impact factors, which provide a valuable basis for the improvement of preventive and therapeutic system of TB and a valuable evidence for improvement of the effect of DOTS stragedy.Methods:Input tuberculosis data in KuanChen District, ChangChun City during 2004-2008 from Changchun City Center for Disease Control into Excel and administer the descriptive statistical analysis; Sampling 100 TB patients, 20 people per year, with simple random sampling method, who were registered and cured in Changchun City Tuberculosis Control Agency during 2004-2008, the questionnaire in the investigation including general characteristics and the implementations of DOTS strategy such as the first diagnostic site, early detection, treatment interruptions , medium-term treatment and the regular check-ups. Use spss13.0 for the general statistical description and analysis of influencing factors.Results:(1)During 2004-2008, the registration rate of active pulmonary tuberculosis in Changchun City Tuberculosis Control Agency ascends from 32.93 to 104.52 per 100,000; The registration rate of smear-positive patients increased from 27.23 to 61.3 per 100,000; The prevalence rate of TB of male is higher than female concentrated in the age of 15 to 45 age group; farmers account for 29.55%. (2) During 2004-2008, the patient load of TB increases every year and 5-year total of outpatients visits is 6,663; the average rate of people load with suspected symptoms is 24.58% per year; the rate of remedy coverage goes up to 90%; the rate of patient following-up increases from 85% to 92.13%; the cure rate of smear positive patients increased from 58.12% to 89.57%. (3) In the survey of 100 patients, the prevalence of knowing-tuberculosis is 72%; the prevalence of regarding TB as a contagious disease is 97%; the top answers of the causes of pulmonary tuberculosis are nutrient-poor (74%), over-exertion (68%) and family economic hardship (53%) by turn. (4) In the survey of 100 tuberculosis patients who totally receive cure, 85% of these patients chose private clinics and village health rooms as the first clinic; 89% of these patients could be diagnosed within three weeks; 39% of these patients ever interrupted treatment, while 32% stopped treatment; 71% of these patients had regular inspections. (5) single-factor analysis shows that female patients are inclined to choose medical institutes with diagnosis ability compared with male(χ~2=4.537, P=0.033); patients with education level above junior middle school are less likely to choose medical institutes with diagnosis ability than those under primary school(χ~2=6.869,P=0.008); patients of different incomes have different rates to medical institutes with diagnosis ability; the rate of choosing medical institutes with diagnosis ability of patients near medical institutes is lower than those far away; the patients'cogntion of TB is one impact factor of stopping treatment(χ~2=5.047,P=0.025).Conclusions:①During 2004-2008,the register rate and smear rate of TB patients in KuanCheng District show increasing trendency. Male patients are more than female, mainly at the age of 15-45 years. Farmer patients account the most, and cumulate in FenJun Xiang, LanJia Town, XingYe Street.②The awareness rate of tuberculosis and entire treatment are low; more health education and prevention propaganda should be enhanced, to improve patients'awareness rate and treatment compliance.③The first sites patients choose are mostly medical institute without diagnosis ability. The rate of non-diagnosed in three weeks is 11%; 39% treatments are interrupted; 32% are stopped; 71% patients had regular inspections. This situation shows the implementation of DOTS stragedy is ordinary.④The main influencing factors for patients to choose first diagnostic site include sex, education level, family income and health service availability; the impact factor of therapy interruption is patients'cognition of TB.⑤To improve the prevention and treatment of TB in KuanCheng District, we should execute more health education for patients and public, take local control and setting up inspiration mechanism to elevate patients'treatment compliance and improve the effect of DOTS to prevent and cure TB from all aspects.
