Objective:Evaluate the effects feasibility and indications of hepatectomy in combination with portaazygous and splenectomy for patients with hepatic carcinoma accompanied by portal hypertension by analyze the clinical data of 43 payients.Methods:The clinical data of 44 patients with hepatic carcinoma accompanied by portal hypertension receiving hepatectomy in combination with portaazygous devascularization and splenectomy from December 2004 to January 2009 in our hospital were retrospectively analyzed.including:1.commonlydata:age,gender.2,tum-orcondition: location,size,postoperative pathology.3.Child-Pugh classification.4.postoperative complication.Meanwhile,all the patients were followed up by telephone.Results:No patients died in the operation.One patients died in three weeks after the operation.Postoperative complications developed in 9 patients(20.9%).The absolute 1-,2- and 3-year survival rates were 88.3%,72.1%and 58.1%,respectively Postoperative upper digestive tract hemorrhage developed in 2 patients,18 patients died.Besides one patient died within 30 d after operation.9 patients died of cancer recurrence or metastasis.7 patients died of liver failure,2 patients died of upper digestive tract hemorrhage.Conclusion:Under the conditions of suitable selection of indications,perfect operative procedure and proper perioperative treatments,hepatectomy in combination with portaazygous devascularization and splenectomy is safe and feasible for patients with hepatic carcinoma accompanied by portal hypertensio.
