Vascular cognitive impairment with no dementia (VCIND) may be the most common subtype of the vascular cognitive impairment (VCI).And it has comprised of all the early or mild cognitive impairment dueing to various cerebrovascular deseases.The cognitive lesion which is focal and/or memory is relatively spared is not up to dementia. It can not lead to severely functional impairment. With the time and progression of desease,the most nts of VCIND may develop to vascular dementia (VaD).In the stage of VCIND there are many risk factors which can be intervened, so to early discover cognitive impairment before daily life is effected and to interfere in it may avoid the late stage of VaD.Objective : 1. The study by analysising the imagings of VCIND patients which have incurred cerebral infarction attempts to show the relationship between the diseased region and amount of cerebral infarction and cognitive impairment, and to discover the association of the white matter lesion and cognitive impairment.2.By surveying the level of homocysteic acid(HCY) and acidum folicum and VitB12,the research try to display the relation with cognitive impairment.3.Observing the cerebralvascular risk factors like hypertentsion, diabetes, hyperlipemia, smoking, heart disease et.contribut to the development of VCIND.4. Viewing the neuropsychology character of VCIND.Methods : The 40 patients with VCIND and 30 cases who have been attack by stroke without cognitive impairment, and the 30 healthy all come from Qilu Hospital out-patient or hospitalization during October,2005 to October,2006.Neuropsychology tests just like MMSE were carried on all the people. All of them had undertaken brain magnetic resonance imaging (MRI), homocysteic acid, acidum folicum, VitB12, blood-fat, and serum glucose.Results: The patients with VCIND had more infarction focus than the cases without cognitive impairment; and they also had more severe white matter deseases; there was no deference between two groups; hypertentsion and diabetes were more seen in VCIND; hyperlipemia, smoking, heart disease were same to the two groups; and Neuropsychology tests showed the execute function was impaired seriously.Conclusion: To VCIND the infarction spot were more seen in basal ganglia and frontal lobe; and the amount of infarction was related to cognitive; white matter diseases was an important reason to cognitive; hypertentsion and diabetes were the risk factor of VCIND; but HCY was not; there were no more than one cognitive lesion to VCIND.
