Comparison Of 3 Different Procedures For Macromastia

ObjectiveTo summarize surgical experience of 3 different procedures for macromastia.MethodTo retrospecte and analyse the information about the clinical application of the reduction mammaplasty performed in 43 patients with macromastia and ptosis from November 1993 to December 2008.The operation methods were selected according to the extent of macromastia and ptosis.7 patients underwent double vertical pedicle reduction mammaplasty, 12 patients underwent improved Strombeck reduction mammaplasty and 24 patients underwent double horizontal-central pedicle reduction mammaplasty.ResultAll the symptoms like breast or shoulder pain or inframammary eczema or sleep-disorder can be relieved after the 3 different procedures for macromastia. Double vertical pedicle reduction mammaplasty:5 cases with first intention and good nipple and areola circulation, 1 case with bad nipple and areola circulation and disruption of wound on both side,l case with bad nipple and areola circulation and disruption of wound on the right side,7 cases with dysesthesia and reflex-disorder of nipple and areola,the appearance of these breasts are plump but not upstanding enough;improved Strombeck reduction mammaplasty: 11 cases with first intention and good circulation, sensation and refex of nipple and areola, 1 case with bad circulation, sensation, reflex of nipple and areola because of venous return disorder, 12 cases with plump and upstanding breast;double horizontal-central pedicle reduction mammaplasty:24 cases with good circulation, sensation,reflex of nipple and areola,and with plump and upstanding breast, 1 case with second intention because of high tension along the wound.Conclusion1.Reduction mammaplasty is the best option for the treatment of macromastia; 2.The circulation, sensation ,refex of nipple and areola are in a poor state in the double vertical pedicle reduction mammaplasty cases,the other 2 procedures can certify the circulation, sensation,refex of nipple and areola;3.Double vertical pedicle reduction mammaplasty get the worst appearance of breast and the most manifest scar,on the other side both improved Strombeck and double horizontal-central pedicle reduction mammaplasty get good appearance of breast and hidden scar,but the latter get more nature appearance and shorter scar;4.In comprehensive consideration, double horizontal-central pedicle reduction mammaplasty is an ideal procedure at present.
