Objective: To study the impact of Xuebijing treatment on inflammatory cytokines in patients with type 2 diabetic nephropathy, and to evaluate the protect role of Xuebijing treatment.Methods: 80 individuals were divided to 3 groups. Group 1 (DN)included 30 type 2 diabetes mellitus patients with microalbuminuria, and group 2 (DM) included 30 type 2 diabetes without microalbuminuria, while group 3(NC) was normal control group included 20 healthy individuals. Their glycosylated hemoglobin(HbA1c), urinary albumin (UALB) and urine creatinine for 24 hours,β2-microglobulin(β2-MG) of serum and urine, serum creatinine(Scr), blood urea nitrogen(BUN), uric acid(UA), cholesterin(CHOL), triglyeride(TG) were measured before and after treatment.Plasminogen activator inhibitor-1(PAI-1),transforming growth factor beta( TGF-β), regulated upon activation normal T cell expresed and secreted(RANTES) and interleukin-8 (IL-8) of urine were also detected in all those individuals.Results: (1)The serum levels of PAI-1, TGF-βand the urine level of RANTES, IL-8, were increased by turns in group NC, group DM, and group DN, and the difference was significant. Correlation analysis showed that the levels of PAI-1, TGF-β, RANTES, IL-8which were positively connected with each other, were positively related with HbA1c, the course of DM and UALB/CR.(2)baPWV and 24-hour urinary albumin significantly positively correlated, but the DM, DN groups baPWV before and after treatment showed no statistical significance.(3)The levels of cholesterin(CHOL), triglyeride(TG), blood rheology and PAI-1, TGF-βof serum, RANTES, IL-8 of urine as well as UALB/CR significant decreased after treatment in above-mentioned two groups; And interclass analysis showed significant difference between two groups.Conclusion:(1) Inflammatory cytokines such as PAI-1, TGF-β, RANTES, IL-8 participated the development of DN, and they had coordinated with each other in DN.(2) baPWV is significantly higher in DN group than in DM group, suggesting that patients with type 2 diabetes and urinary albumin in the arterial wall related function, that is, with early atherosclerosis is closely related.(3) Xuebijing has the functions of anti-inflammation and renal protection. It can not only decrease cholesterin, triglyeride and improve hypercoagulabale state, but also restrain the inflammation process, lessen the ECM and delay the development of DN.
