Objective Cervical cancer is most common woman genital system malignant tumor second only breast cancer, and showing young tendency. Although the incidence of cervical cancer has decreased notably, the long term survival rate is lower. For sake of cutting down the incidence and mortality, it is critical for screening precancerous lesion and detecting prognosis. The purpose of this study is to detect the expression of the hTERC and HER-2 oncogene in uterine cervix with different pathological changes, and to evaluate the effect and clinical significance of hTERC and HER-2 in cervical cancer.Methods The normal cervical scraping samples from the clinic Physical Check-Ups women who have been examinated showing normal and the exfoliocytology examination also showing negative between 2007 and 2008 in Department of Obstetrics & Gynecology, Anhui Provincical Hospital.Use the fluorescent in situ hybridization(FISH) to detect the expression of HER-2 and hTERC oncogene in normal cervixes, and establish the amplification threshold of the hTERC and HER-2 oncogene. The Scraping Samples have been selected from 15 normal cases with chronic cervicitis and from 76 cases with cervical intraepithelial neoplasia and from 24 cases with infiltrating cervical cancer. All cases have TCT and pathological examination and have not been cured. The aim is to detect the expression of the hTERC and HER-2 oncogene. Use the Immunohistochemistry (IHC) to detect the the expression of hTERT and P185c-erbB-2 in pathological section of these cases simultaneously.Our study is to evaluate the function and Clinical Significance of hTERC and HER-2 gene in the process of cervical carcinogenesis.Results①Establish the amplification threshold of the hTERC and HER-2 oncogene: detect the expression of hTERC and HER-2 oncogene in 15 normal normal cervixes, the cells which contain not less than two green signals and exceed two red signals have been calculated, the threshold is equal to mean plus tre-standard deviation. The threshold of the hTERC and HER-2 oncogene is equal to 3.78% and 5.12%.That is, if the cells contain more than two red signals exceed the threshold, they were judged positive.②Positive expression rates of the hTERC gene in normal cervixes, CINⅠ, CINⅡ/Ⅲand infiltrating cervical cancer were 0%, 18.8%, 65% and 100%. Compared with control cervix and CIN, hTERC gene positive expression in infiltrating cervical cancer was different significantly (p<0.05), and there was a significantly difference between CINⅡ/Ⅲand control cervix(p<0.05).③Positive expression rates of HER-2 in normal cervixes, CIN , CINⅡ/Ⅲand infiltrating cervical cancer were 0.0%, 6.3%, 16.6%, 58.3%, respectively. Compared with control cervix and CIN, HER-2 gene positive expression in infiltrating cervical cancer was different significantly (p<0.05), and there was a significantly difference between CINⅡ/Ⅲand control cervix(p<0.05).④The over expression of HER-2 was correlated with the differentiation degree of carcinoma closely (p<0.05). NO correlation was found between positive expression and clinical stages, nor pathologic types.⑤Positive expression rates of P185c-erbB-2 in normal cervixe, CINⅠ, CINⅡ/Ⅲand infiltrating cervical cancer were 0.0%, 6.3%, 16.6%, 45.8% by IHC in pathological section, respectively. The equivalent positive expression rates of the hTERT were 0%, 18.8%, 75% and 100%. There was no significant difference between CINⅠand CINⅡ/Ⅲ(P>0.05). Compared with control cervix,positive expression of the HER-2 and hTERT in CINⅡ/Ⅲand infiltrating cervical cancer were different significantly (P<0.05), and the expression in CIN was lower than that in infiltrating cervical cancer (P<0.05).Conclusions The positive expression and amplification tendency of hTERC and HER-2 gene have presented upgrade in uterine cervix with the advancement of pathological changes. Compared with control cervix and CINⅡ/Ⅲ, there was a significantly difference in cervical cancer. The over expression of HER-2 was correlated with the differentiation degree of carcinoma closely. NO correlation was found between positive expression and clinical stages, nor pathologic types. The over expression of hTERC and HER-2 gene may play an important role in cervical carcinogenesis, the hTERC and HER-2 gene of pathological changes could be used as a better tumor index for early cervical cancer screening and predicate the the advancement and evaluate poor prognosis of cervical leision.
