Objective: To explore the clinical diagnosis, surgical approach via improved pterional way and the skills in microneurosurgery of the invasive pituitary adenomas.Methods: 60 cases with Pituitary adenoma which invades beyond the sellar region who had received microneurosurgery from March 2008 to March 2009 were analyzed retrospectively. All cases of Pituitary adenoma were confirmed through MRI exams and underwent microneurosurgical resection via improved pterional way. Posto-peration MRI exam were executed on all patients .Results: Through microscope, 59 lesions are totally resected, 1 lesions are department removed. The common complication after surgery was hypopituitarism and diabetes insipidus.Conclusions: CT and MRI are valuable for the diagnosis of the Pituitary adenomas. Microneurosurgery via improved pterional way is the safety and effective treatment for the invasive Pituitary adenomas. Appropriate operative approaches and skilled microneuro -surgical techniques based on the type, location and adjacent relationship, to reveal sufficient chiasm opticum and stalk hypophysial and three pavimentum ventriculi,carefully distinguish the complication with coma and hypothalamus and appendix cerebri,sharply abstracet coma are the keys of success in the treatment.
