Objective: To obtain the detection rate of esophageal cancer and every arrange of precancerous lesion in the high risk area of Feicheng City Shandong Province by esophageal cancer screening; and to investigate the awareness of esophageal cancer and the perception of payment in screening population. It can provide useful information for further study in esophageal cancer screening and early detection and treatment projects in this region.Methods: In Feicheng City, Shandong Province, using random cluster sampling method select study objects for epidemiological survey and endoscopic screening. The subjects must not have clinical symptom and age from 40 to 69. Before one to one epidemiological survey, all the objects must sign the informed consent. The questionnaire included: baseline information, risk factor of esophageal cancer and screening awareness and willingness of esophageal cancer and so on. We use iodine staining of endoscopic-assisted and biopsy methods for esophageal endoscopy in the target population that is non-endoscopic screening. If the result is abnormity, we will get to use biopsy for further examination, then use the final histopathological results as gold standard for diagnosis. All the epidemiological survey and screening results are based on Visual FoxPro database to carry out the procedures for entry and management, all the information recorded twice, then using the Epidata to carry data consistency tests, after that we organized database into the final database. Using SPSS11.5 for statistical analysis, statistical methods isχ2 test,χ2 trend test.Results: Among 2006-2008, the esophageal cancer and all levels of precancerous lesions detection analysis showed that the detection rate of three-years differences were significant, but no upward trend by years. Dividing 5 years old as one group, different age groups esophageal detection screening analysis showed that esophageal lesions rate is in- creasing with the age growth yearly. The different lesions of esophageal have different increasing trends with age growth, esophagitis, basal cell hyperplasia, mild dysplasia, detection rate increased gradually with age, moderate and severe dysplasia,detection rate in the 55-59 age group is up to the highest, mucosal carcinoma, detection rate in the 50-54 age group is up to the highest. Gender at esophageal cancer and all levels of precancerous lesion detection analysis showed that levels of male esophageal cancer and precancerous lesion detection rate were higher than women, and moderate dysplasia and invasive cancer, all levels of precancerous lesions detective rate are statistically significance byχ2 test. The analysis of esophageal cancer screening purpose and results showed that awareness are more than 99%; an analysis of the wishes of the screening and results showed that willingness who want to pay a certain cost reach 90.82%, supporting 50 yuan below to screening, the proportion is up to 58.97%; willing to participate in the screening but do not want to pay only 9.18% and due to economic difficulties and thinking is not necessary to participate in the screening so that they do not want to pay, the proportion is 67.39% and 21.74%.Conclusions: Feicheng City, Shandong Province esophageal lesions detection rate at all levels are higher; the detection rate of esophageal lesions has age and sex characteristics, with age growth, the detection rate of esophageal lesions increase , gender detection rate of esophageal lesions are different that male is higher than female. Local residents have a higher awareness of esophageal cancer and the purpose of screening, at same time, the screening will and willingness to pay for screening is relatively high, for that it has a good foundation for early detection and treatment. In order to reduce the incidence of esophageal cancer and mortality, we should carry out further screening.
